"Irreversible Bruton's tyrosine kinase inhibitor obrutinib in primary immune thrombocytopenia (ITP) : results of a randomized, open-label phase II study.

The study was designed to evaluate the efficacy, safety, and tolerability of obrutinib in patients with persistent or chronic primary ITP. The primary end point was the percentage of patients who had a platelet count of at least 50x109 per liter on at least two consecutive occasions and who had not used rescue therapy in the previous 4 weeks.

A total of 33 patients were enrolled as of February 6, 2023. Obrutinib showed a favorable safety profile in both groups of ITP patients treated with 50mg and 30mg once daily oral obrutinib. 50mg once daily has a rapid onset of action and is better, especially in patients who have previously responded to glucocorticoid (GC)/ intravenous immunoglobulin (IVIG) therapy.

A total of 36.4% (12 of 33) of the patients met the primary end point and 40% (6 of 15) of the patients in the 50-mg group met the primary end point. Of the 12 patients who met the primary end point, 83.3% (10 of 12) had a sustained response (defined as a platelet count of ≥50x109 per liter on at least four of six visits between weeks 14 and 24).

Among 22 patients with prior GC or IVIG response, 75.0% in the 50-mg group met the primary endpoint.

Obrutinib was safe and well tolerated in the treatment of ITP, and treatment-related adverse events (TRAEs) were grade 1 or 2.

ORIENT study: Obrutinib combined with R-CHOP as first-line treatment for patients with non-GCB diffuse large B-cell lymphoma (DLBCL)

The aim of this study is to investigate the efficacy and safety of obrutinib combined with R-CHOP regimen in newly diagnosed patients with non-germinal center B-cell-like (non-GCB) diffuse large B-cell lymphoma (DLBCL) after receiving induction therapy with obrutinib combined with rituximab.

This preliminary study shows that obrutinib combined with rituximab can achieve a high response rate in the short-term induction treatment of newly diagnosed non-GCB DLBCL patients. Patients who benefited from the obrutinib-rituximab regimen achieved a higher complete response rate and demonstrated a favorable safety profile when they continued to receive obrutinib plus R-CHOP.

Preliminary results of a phase II study of pomalidomide, obrutinib and rituximab combined with no chemotherapy followed by high-dose methotrexate in patients with newly diagnosed primary central nervous system lymphoma (PCNSL)

This is the first study of a targeted combination therapy prior to chemotherapy in newly diagnosed primary central nervous system lymphoma (PCNSL). The combination of pomalidomide, obrutinib, and rituximab demonstrated a high overall response rate (ORR) and good tolerability, suggesting the potential for noncytotoxic first-line treatment of PCNSL.
