Memantine is a low to moderate affinity noncompetitive NMDA receptor antagonist that preferentially binds to cation channels operated by NMDA receptors. There is no evidence that memantine prevents or slows neurodegenerative changes in patients with Alzheimer's disease. Memantine has low affinity for GABA, benzodiazepine, dopamine, adrenergic, histamine, glycine receptors, and voltage-dependent Ca2, Na, and K channels. Memantine can also antagonize 5HT3 receptors with potency similar to that of NMDA receptors, and its potency at antagonizing nicotinic acetylcholine receptors is 1/6 to 1/10 of that of NMDA receptors. In vitro studies have shown that memantine does not affect the reversible inhibition of acetylcholinesterase by donepezil, galantamine, or tacrine.
