1. Effects on water and electrolyte excretion: diuretic effect, increased excretion of urinary sodium, potassium, chloride, phosphorus and magnesium, and reduced excretion of urinary calcium; inhibiting the reabsorption of sodium chloride in the distal tubule proximal and proximal tubules, and increasing the Na-K exchange in the distal tubules and collecting ducts. It may work by inhibiting the activity of carbonic anhydrase and phosphodiesterase. 2. Antihypertensive effect: In addition to diuresis and sodium excretion, there may be extrarenal mechanisms involved, such as increased gastrointestinal excretion of Na. 3. Effects on renal hemodynamics and glomerular filtration function: Reduced reabsorption of water and Na by the renal tubules leads to increased intratubular pressure, stimulating the macula densa through tubule-glomerular reflex, increasing the secretion of renin and angiotensin in the kidney, causing renal vasoconstriction, and decreased renal blood flow and glomerular filtration rate.
