Function and Efficacy

This product is a proton pump inhibitor of gastric parietal cells. It can specifically inhibit the H and K-ATPases on the secretory microtubules composed of the apical membrane of parietal cells and the tubular vesicles in the cytoplasm, thereby effectively inhibiting the secretion of gastric acid. Since H, K-ATPase is the last process of acid secretion by parietal cells, this product has strong acid-suppressing ability. It can not only non-competitively inhibit gastric acid secretion caused by gastrin, histamine, choline and food, stimulate the vagus nerve, etc., but also inhibit part of the basic gastric acid secretion that is not affected by choline or H2 receptor blockers. It also has a strong and lasting inhibitory effect on gastric acid secretion caused by dibutyl cyclic adenosine monophosphate (DcAMP) stimulation that cannot be inhibited by H2 receptor antagonists. This product also has an inhibitory effect on pepsin secretion, has no obvious changes in gastric mucosal blood flow, and does not affect body temperature, gastric cavity temperature, arterial blood pressure, venous hemoglobin, arterial oxygen partial pressure, carbon dioxide partial pressure and arterial blood pH.

Ingredients

Appearance

Indication

Mainly used for: ① bleeding from peptic ulcer and anastomotic ulcer. ② acute gastric mucosal damage during stress and acute gastric mucosal damage caused by nonsteroidal anti-inflammatory drugs; ③ also commonly used to prevent severe diseases (such as cerebral hemorrhage, severe trauma, etc.) and prevent rebleeding after gastric surgery; ④ prevent gastric acid reflux combined with aspiration pneumonia after general anesthesia or major surgery and in patients with weakness and coma.

Usage and Dosage

1. Intravenous drip. 2. Dissolve the contents of the bottle in 100 ml of 0.9% sodium chloride injection or 100 ml of 5% glucose injection before use. After the product is dissolved, the intravenous drip time should be 20-30 minutes or longer. 3. When oral therapy is not suitable for patients with duodenal ulcer, gastric ulcer and reflux esophagitis, the recommended intravenous drip dose of this product is 40 mg, once a day. 4. For patients with Zollinger-Ellison syndrome, intravenous drip of omeprazole sodium 60 mg is recommended as the starting dose, once a day. The daily dose may require a higher dose, and the dose should be individualized. When the daily dose exceeds 60 mg, it should be administered in two doses.

Adverse Reactions

Occasionally, transient mild nausea, diarrhea, abdominal pain, paresthesia, dizziness or headache may occur, but it will not affect the treatment.

Special Population Medication

Precautions for children: There is no experience of using this product for children. Precautions for pregnancy and lactation: Although animal experiments have not found that this product has adverse effects on pregnancy and lactation or toxic or teratogenic effects on the fetus, it is recommended that pregnant and lactating women should avoid using it as much as possible. Precautions for the elderly: No dosage adjustment is required for elderly patients.

Drug Interactions

Because this product can reduce gastric acidity, the absorption of some drugs may be changed. Therefore, the absorption of ketoconazole and itraconazole will decrease when treated with omeprazole or other acid inhibitors or antacids. Because omeprazole is metabolized in the liver by cytochrome F4502C19 (CYP2C19), it will prolong the clearance of other enzymatic products such as diazepam, warfarin (R-warfarin) and phenytoin. This product should be monitored when used with warfarin and phenytoin, and the dose of warfarin or phenytoin should be reduced if necessary. Patients who continue to use phenytoin are given 20 mg of this product once a day, and the blood concentration of phenytoin is not affected. Similarly, patients who continue to use warfarin are given 20 mg of this product once a day, and the clotting time is not changed. When omeprazole is used in combination with clarithromycin, their blood concentrations will increase. However, there is no interaction when used in combination with metronidazole or amoxicillin. These antibiotics combined with omeprazole can eradicate H. pylori.
