Function and Efficacy

This product is a blood volume expander. After intravenous injection, it can increase plasma colloidal osmotic pressure, absorb extravascular water to increase blood volume, and increase and maintain blood pressure. Its blood volume expansion effect is weaker and shorter than that of dextran 70, but its effect on improving microcirculation is stronger than that of dextran 70. It can disaggregate aggregated red blood cells and platelets, reduce blood viscosity, improve microcirculation, and prevent thrombosis. In addition, it also has an osmotic diuretic effect.

Ingredients

This product is a sterile aqueous solution of dextran 40 and 0.9% sodium chloride. Dextran 40 is a high molecular weight glucose polymer produced by fermentation of sucrose, with an average molecular weight of 40,000.

Indication

1. Shock is used for shock and toxic shock caused by various reasons such as blood loss, trauma, burns, etc. 2. Prevention of postoperative venous thrombosis is used for limb replantation and vascular surgery to prevent postoperative thrombosis. 3. Vascular embolic diseases are used for angina pectoris, cerebral thrombosis, cerebral blood supply insufficiency, thromboangiitis obliterans, etc. 4. During extracorporeal circulation, it replaces part of the blood and pre-fills the artificial heart-lung machine, which can save blood and improve circulation.

Usage and Dosage

Intravenous drip, the dosage depends on the condition. The usual dosage for adults is 250-500ml at a time, and no more than 1,000-1,500ml within 24 hours. The dosage for infants is 5ml/kg, and the dosage for children is 10ml/kg. Shock cases: The dosage can be larger and the speed can be faster. The drip speed is 20-40ml/min. The maximum dose on the first day can be up to 20ml/kg. Dehydration must be corrected before use. Prevention of postoperative thrombosis: 500ml is given during or after surgery, usually 500ml/day on the first and second days after surgery, and dripped at a speed of 2-4 hours. For high-risk patients, the course of treatment can be up to 10 days. Vascular embolic diseases: It should be dripped slowly, generally 250-500ml each time, once a day or every other day, and 7-10 times is a course of treatment.

Adverse Reactions

1. Allergic reaction A small number of patients may experience allergic reactions, manifested as skin itching, urticaria, nausea, vomiting, asthma, and in severe cases, cyanosis of the lips, collapse, a sharp drop in blood pressure, bronchospasm, and some patients may even experience anaphylactic shock and even death. The incidence of allergic reactions is about 0.03-4.7%. People with allergic constitutions should undergo skin tests before use. 2. Fever, chills, swollen lymph nodes, arthritis, etc. are occasionally seen. 3. Bleeding tendency can cause coagulation disorders and prolong bleeding time. This reaction is often related to the dose.

Precautions

1. Contraindicated for patients with congestive heart failure and other hypervolemia. 2. Contraindicated for patients with severe thrombocytopenia, coagulation disorders and other bleeding. 3. Use with caution in patients with poor heart, liver and kidney function; contraindicated for patients with oliguria or anuria. 4. Use with caution in patients with active pulmonary tuberculosis. 5. Use with caution in patients with a history of allergies. Contraindicated for patients with oliguria or anuria

Drug Interactions

1. When used in combination with heparin, the possibility of bleeding increases due to synergistic effects. 2. When used in combination with gentamicin and paromomycin, nephrotoxicity increases.
