Function and Efficacy

This product belongs to the aminoglycoside antibiotics. The antibacterial spectrum is similar to that of gentamicin. It has antibacterial effects on Gram-negative bacteria such as Escherichia coli, aerogenes, Klebsiella, Proteus mirabilis, some indole-positive Proteus, Pseudomonas aeruginosa, some Neisseria, some non-pigmented Serratia and Shigella; the antibacterial effect of this product on Pseudomonas aeruginosa is 3 to 5 times stronger than that of gentamicin, and Pseudomonas aeruginosa that is moderately sensitive to gentamicin is highly sensitive to this product. Among Gram-positive bacteria, Staphylococcus aureus (including (lactamase-producing strains) is sensitive to this product; Streptococci (including Streptococcus pyogenes, Pneumococcus, Streptococcus faecalis, etc.) are all resistant to this product. Anaerobic bacteria (Bacteroides), Mycobacterium tuberculosis, Rickettsia, viruses and fungi are also resistant to this product. The mechanism of action of this product is to bind to the 30S subunit of the bacterial ribosome and inhibit the synthesis of bacterial proteins.

Ingredients

Indication

This product is suitable for neonatal sepsis, septicemia, central nervous system infection (including meningitis), urogenital system infection, lung infection, biliary tract infection, abdominal infection and peritonitis, bone infection, burns, skin and soft tissue infection, acute and chronic otitis media, sinusitis, etc. caused by Pseudomonas aeruginosa, Proteus, Escherichia coli, Klebsiella, Enterobacter, Serratia, or combined with other antibacterial drugs for staphylococcal infection (methicillin-resistant strains are ineffective). This product can be administered by intrathecal injection when used for Pseudomonas aeruginosa meningitis or ventriculitis; it can be inhaled by aerosol as an auxiliary treatment when used for bronchial and lung infections.

Usage and Dosage

Adults: 80-240 mg/day, divided into 2-3 injections; children: 3-5 mg/kg body weight/day, divided into 2-3 injections, intramuscular or intravenous.

Adverse Reactions

1. Systemic administration combined with intrathecal injection may cause leg cramps, rash, fever and whole body spasms. 2. Patients with a higher incidence rate have hearing loss, tinnitus or fullness in the ears (ototoxicity), hematuria, significantly reduced urination frequency or decreased urine volume, loss of appetite, extreme thirst (nephrotoxicity), unsteady gait, and dizziness (ototoxicity, vestibular effects, and nephrotoxicity). Patients with a lower incidence rate have difficulty breathing, drowsiness, and extreme weakness (neuromuscular blockade or nephrotoxicity). The incidence of renal dysfunction caused by this product is lower than that of gentamicin. 3. If hearing loss, tinnitus or fullness in the ears occurs after discontinuation of the drug, pay attention to ototoxicity.

Precautions

1. Patients who are allergic to this product or other aminoglycosides, or who have family members who have developed deafness or other hearing loss due to the use of streptomycin are contraindicated. 2. Patients with renal failure are contraindicated.

Drug Interactions

1. This product can increase ototoxicity, nephrotoxicity and neuromuscular blocking effects when used in combination with other aminoglycosides or when applied topically or systemically one after the other. Hearing loss may occur, and may still progress to deafness after discontinuation of the drug; hearing damage may recover or become permanent. Neuromuscular blocking effects can lead to skeletal muscle weakness, respiratory depression or respiratory paralysis (apnea), and anticholinesterase drugs or calcium salts can help restore the blocking effect. 2. This product can aggravate neuromuscular blocking effects when used in combination with neuromuscular blocking drugs, leading to muscle weakness, respiratory depression or respiratory paralysis (apnea). When used in combination with plasma substitutes such as dextran, sodium alginate, diuretics such as ethacrynic acid, furosemide, capreomycin, cisplatin, vancomycin, etc., or when applied topically or systemically one after the other, ototoxicity and nephrotoxicity may be increased, and hearing damage may occur, and may still progress to deafness after discontinuation of the drug, and hearing damage may recover or become permanent. 3. This product can increase nephrotoxicity when used topically or systemically with cephalothin. 4. This product should not be used in combination with polymyxins, or applied topically or systemically in sequence, because it can increase nephrotoxicity and neuromuscular blocking effects, the latter of which can lead to skeletal muscle weakness, respiratory depression or respiratory paralysis (apnea). 5. This product should not be used in combination with other nephrotoxic or ototoxic drugs, or used sequentially, so as not to aggravate nephrotoxicity or ototoxicity. 6. This product can often achieve synergistic effects when used in combination with beta-lactams (cephalosporins or penicillins). 7. This product can be mixed with beta-lactams (cephalosporins or penicillins) to cause mutual inactivation. When used in combination, it must be dripped in separate bottles. This product should not be dripped in the same bottle with other drugs.
