Paclitaxel Injection
Function and Efficacy
This product is a new type of anti-microtubule drug. It promotes tubulin polymerization, inhibits depolymerization, maintains tubulin stability, and inhibits cell mitosis. In vitro experiments have shown that paclitaxel has a significant radiosensitization effect, which may be to stop cells in the G2 and M phases that are sensitive to radiotherapy. Paclitaxel is a mitotic inhibitor or spindle poison. It has a different mechanism of action from the commonly used chemotherapy drugs. It induces and promotes the assembly of microtubules. Paclitaxel has the effect of polymerizing and stabilizing microtubules, causing rapidly dividing tumor cells to be firmly fixed in the mitotic stage, blocking the replication of cancer cells and causing them to die.

Indication
First-line chemotherapy for metastatic ovarian cancer after initial or subsequent chemotherapy failure. Metastatic breast cancer after combined chemotherapy failure or recurrence within 6 months of adjuvant chemotherapy, first-line treatment for advanced non-small cell lung cancer, and adjuvant therapy after surgery for lymph node-positive breast cancer.
Usage and Dosage
To prevent allergic reactions, take dexamethasone 10 mg orally 12 hours before paclitaxel treatment, and take dexamethasone 10 mg orally 6 hours before treatment. Give diphenhydramine 20 mg intramuscularly and cimetidine 300 mg or ranitidine 50 mg intravenously 30 to 60 minutes before treatment. The single-drug dose is 135 to 200 mg/m2, and with G-CSF support, the dose can reach 250 mg/m2. Dilute paclitaxel with normal saline or 5% glucose saline and drip it intravenously for 3 hours. The combined dose is 135 to 175 mg/m2, repeated every 3 to 4 weeks.
Adverse Reactions
1. Allergic reactions: The incidence rate is 39%, of which severe allergic reactions occur in 2%. Most are type 1 allergic reactions, manifested as bronchospasm, dyspnea, urticaria and hypotension. Almost all reactions occur within the first 10 minutes after medication. 2. Bone marrow suppression: It is the main dose-limiting toxicity, manifested as neutropenia, and thrombocytopenia is rare, generally occurring 8 to 10 days after medication. The incidence of severe neutrophilia is 47%, and the incidence of severe thrombocytopenia is 5%. Anemia is more common. 3. Neurotoxicity: The incidence of peripheral neuropathy is 62%, the most common manifestations are mild numbness and paresthesia, and the incidence of severe neurotoxicity is 6%. 4. Precautions
People who are allergic to polyoxyethyl castor oil. Contraindicated for patients with neutrophil count less than 1.5/10^9/L and severe bone marrow suppression.
Special Population Medication
Precautions for children: Precautions for pregnancy and lactation: Paclitaxel has been shown to affect embryonic growth in animal experiments, so it is contraindicated for pregnant women. Women of childbearing age should not become pregnant during treatment. Precautions for the elderly:
Drug Interactions
Pharmacokinetic data show that the clearance of paclitaxel injection is reduced by about 30% when cisplatin is given after administration, and bone marrow toxicity is more serious. Simultaneous administration of ketoconazole affects the metabolism of paclitaxel injection.
