Dexamethasone Acetate Tablets

Function and Efficacy

Adrenal cortical hormone drugs have more significant anti-inflammatory, anti-allergic and anti-shock effects than prednisone, but have little effect on water and sodium retention and promoting potassium excretion, and have a strong inhibitory effect on the pituitary-adrenal gland. 1 Anti-inflammatory effect: This product can reduce and prevent tissue response to inflammation, thereby reducing the manifestations of inflammation. Hormones inhibit the accumulation of inflammatory cells, including macrophages and leukocytes at the site of inflammation, and inhibit phagocytosis, the release of lysosomal enzymes, and the synthesis and release of inflammatory chemical mediators. It can reduce and prevent tissue response to inflammation, thereby reducing the manifestations of inflammation. 2 Immunosuppressive effect: including preventing or inhibiting cell-mediated immune responses, delayed allergic reactions, reducing the number of T lymphocytes, monocytes, and eosinophils, reducing the ability of immunoglobulins to bind to cell surface receptors, and inhibiting the synthesis and release of interleukins, thereby reducing the transformation of T lymphocytes into lymphoblasts and reducing the expansion of primary immune responses. It can reduce the passage of immune complexes through the basement membrane and reduce the concentration of complement components and immunoglobulins.

Appearance

This product is white tablets.

Indication

It is mainly used for allergic and autoimmune inflammatory diseases, such as connective tissue diseases, severe bronchial asthma, allergic diseases such as dermatitis, ulcerative colitis, acute leukemia, malignant lymphoma, etc. In addition, this drug is also used for the diagnosis of certain adrenal cortical diseases - dexamethasone suppression test.

Usage and Dosage

For oral administration, the starting dose for adults is 0.75-3.00 mg (1-4 tablets) once, 2-4 times a day. The maintenance dose is about 0.75 mg (1 tablet) per day, depending on the condition.

Adverse Reactions

Large doses of this product can easily cause diabetes, peptic ulcers and Cushing's syndrome-like symptoms, and have a strong inhibitory effect on the hypothalamus-pituitary-adrenal axis. Concurrent infection is the main adverse reaction.

Precautions

Patients with a history of allergy to this product and adrenal cortex hormones are contraindicated. Patients with hypertension, thrombosis, gastric and duodenal ulcers, mental illness, electrolyte metabolism abnormalities, myocardial infarction, visceral surgery, glaucoma, etc. are generally not suitable for use. Weigh the pros and cons in special circumstances, but be aware of the possibility of worsening of the condition.

Special Population Medication

Precautions for children: Children should be very cautious when using adrenocortical hormones. The use of hormones can inhibit the growth and development of children. If long-term use is necessary, short-acting or medium-acting preparations should be used, and long-acting dexamethasone preparations should be avoided. Changes in intracranial pressure should also be observed. Precautions for pregnancy and lactation: Pregnant women may increase the incidence of placental insufficiency, neonatal weight loss, or stillbirth. Animal experiments have teratogenic effects, so the pros and cons should be weighed before use. If a nursing mother receives a large dose of the drug, she should not breastfeed to prevent the drug from being excreted through breast milk, causing adverse reactions such as infant growth inhibition and adrenal function inhibition. Precautions for the elderly: It is easy to cause hypertension. Elderly patients, especially postmenopausal women, are prone to osteoporosis when using it.

Drug Interactions

1. When taken with barbiturates, phenytoin, and rifampicin, the metabolic promoting effect of this product is weakened. 2. When used with salicylic acid drugs, their toxicity is increased. 3. It can weaken the effects of anticoagulants and oral hypoglycemic drugs, and the dosage should be adjusted. 4. When used with diuretics (except potassium-sparing diuretics), it can cause hypokalemia, so the dosage should be noted.
