Function and Efficacy

Adrenal cortical hormones have anti-inflammatory, anti-allergic, anti-rheumatic and immunosuppressive effects. The mechanism of action is as follows: 1. Anti-inflammatory effect: This product can reduce and prevent tissue response to inflammation, thereby reducing the manifestation of inflammation. Hormones inhibit the accumulation of inflammatory cells, including macrophages and leukocytes at the site of inflammation, and inhibit phagocytosis, the release of lysosomal enzymes, and the synthesis and release of inflammatory chemical mediators. 2. Immunosuppressive effect: including preventing or inhibiting cell-mediated immune responses, delayed allergic reactions, reducing the number of T lymphocytes, monocytes, and eosinophils, reducing the ability of immunoglobulins to bind to cell surface receptors, and inhibiting the synthesis and release of interleukins, thereby reducing the transformation of T lymphocytes into lymphoblasts and reducing the expansion of primary immune responses. Appearance

This product is white tablets.

Indication

It is mainly used for allergic and autoimmune inflammatory diseases. It is suitable for connective tissue diseases, systemic lupus erythematosus, severe polymyositis, severe bronchial asthma, dermatomyositis, vasculitis and other allergic diseases, acute leukemia, malignant lymphoma and other diseases suitable for other adrenal cortex hormone drugs.

Usage and Dosage

1 Oral administration: generally 5-10 mg (1-2 tablets) at a time, 10-60 mg (2-12 tablets) a day. 2 For autoimmune diseases such as systemic lupus erythematosus, gastropathic syndrome, ulcerative colitis, autoimmune hemolytic anemia, etc., 40-60 mg can be given daily, and the dosage can be gradually reduced after the condition stabilizes. 3 For allergic diseases such as drug-induced dermatitis, urticaria, bronchial asthma, etc., prednisone can be given at 20-40 mg daily, and the dosage can be reduced after the symptoms are relieved, reducing by 5 mg every 1-2 days. 4 To prevent organ transplant rejection, generally 100 mg is taken orally daily starting 1-2 days before surgery, and 60 mg is changed to daily one week after surgery, and then gradually reduced. 5 For the treatment of acute leukemia and malignant tumors, 60-80 mg is taken orally daily, and the dosage can be reduced after the symptoms are relieved.

Adverse Reactions

Large doses of this product can easily cause diabetes, peptic ulcers and symptoms similar to Cushing's syndrome, and have a strong inhibitory effect on the hypothalamus-pituitary-adrenal axis. Concurrent infection is the main adverse reaction.

Precautions

It is not suitable for patients with hypertension, thrombosis, gastric and duodenal ulcers, mental illness, abnormal electrolyte metabolism, myocardial infarction, visceral surgery, glaucoma, etc. It is contraindicated for patients with a history of allergy to this product and adrenal cortical hormones, and for those with fungal and viral infections.

Special Population Medication

Precautions for children: Children should use with caution. Precautions for pregnancy and lactation: Pregnant and lactating women should use with caution. Precautions for the elderly: The use of glucocorticoids can easily cause high blood pressure, and elderly patients, especially postmenopausal women, are prone to osteoporosis.

Drug Interactions

1. Nonsteroidal anti-inflammatory analgesics can enhance its ulcerogenic effect. 2. It can enhance the hepatotoxicity of acetaminophen. 3. When used in combination with amphotericin B or carbonic anhydrase inhibitors, it can aggravate hypokalemia. Long-term use with carbonic anhydrase inhibitors can easily lead to hypocalcemia and osteoporosis. 4. When used in combination with protein anabolic hormones, it can increase the incidence of edema and aggravate acne. 5. Long-term use with anticholinergics (such as atropine) can cause increased intraocular pressure. 6. Tricyclic antidepressants can aggravate the mental symptoms caused by it. 7. When used in combination with hypoglycemic drugs such as insulin, the dose of hypoglycemic drugs should be appropriately adjusted because it can increase blood sugar in diabetic patients. 8. Thyroid hormones can increase its metabolic clearance rate, so when thyroid hormones or antithyroid drugs are used in combination with it, the dose of the latter should be appropriately adjusted. 9. When used in combination with contraceptives or estrogen preparations, it can enhance its therapeutic effects and adverse reactions. 10. When used in combination with cardiac glycosides, it can increase the occurrence of digitalis toxicity and arrhythmia. 11 When used in combination with potassium-excreting diuretics, it can cause severe hypokalemia and weaken the sodium-excreting and diuretic effects of diuretics due to water and sodium retention. 12 When used in combination with ephedrine, its metabolic clearance can be enhanced. 13 When used in combination with immunosuppressants, it can increase the risk of infection and may induce lymphoma or other lymphoproliferative diseases. 14 It can increase the metabolism and excretion of isoniazid in the liver, and reduce the blood concentration and efficacy of isoniazid. 15 It can promote the metabolism of mexiletine in the body and reduce the blood concentration. 16 When used in combination with salicylates, it can reduce the concentration of plasma salicylate. 17 When used in combination with growth hormone, it can inhibit the latter's growth-promoting effect.
