Function and Efficacy
Citalopram is an antidepressant and a dicyclic hydrogenated phthalide derivative. The antidepressant mechanism of action of Citalopram may be related to inhibiting the reuptake of 5-HT by neurons in the central nervous system, thereby enhancing the function of central serotonergic nerves. In vitro and animal experiments indicate that Citalopram is a highly selective 5-HT reuptake inhibitor with little effect on the reuptake of norepinephrine and dopamine. Rats were given Citalopram for 14 days, and the effect of inhibiting 5-HT uptake was not tolerated. Citalopram is a racemic body, and its effect of inhibiting 5-HT reuptake is mainly exerted by its (S)-enantiomer. Appearance
This product is a film-coated tablet, which appears white or off-white after removing the coating.
Indication
Various types of depression.
Usage and Dosage
Oral, Adults: 20mg to 60mg daily, once a day. Starting from 20mg daily, it can be increased to 60mg, the maximum daily dose, depending on the severity of the disease and the patient's response. The increment needs to be 2-3 weeks apart. It usually takes 2-3 weeks of treatment to determine the efficacy. To prevent relapse, the treatment lasts at least 6 months. No withdrawal symptoms have been reported in clinical trials, but serotonin reuptake inhibitors (SSRIs) may cause withdrawal symptoms, so it takes 1 week of gradual reduction before stopping the drug. For elderly patients over 65 years old and patients with liver damage, the dose is halved, the usual dose is 10-30mg per day, starting from 10mg per day, the recommended usual dose is 20mg per day, and the maximum daily dose is 40mg.
Adverse Reactions
The adverse reactions of this product are usually short-lived and mild. They are usually obvious within the first or second week after taking the medicine, and generally disappear gradually as the depressive symptoms improve. Common adverse reactions include nausea, dry mouth, dizziness, headache, drowsiness, shortened sleep time, sweating, decreased salivation, tremor, and diarrhea. In foreign clinical studies and adverse reactions reported after marketing, dose-related adverse reactions include fatigue, impotence, insomnia, sweating, drowsiness, and yawning. The following rare adverse reaction reports are temporarily believed to be related to this product; angioedema, choreoathetosis, epidermal necrosis, erythema multiforme, hepatic necrosis, antidepressant malignant syndrome, pancreatitis, serotonin syndrome, spontaneous abortion, thrombocytopenia, arrhythmia, priapism, torsade de pointes, withdrawal syndrome
Precautions
This product is contraindicated for those who are allergic to any of the ingredients in this product. This product cannot be used simultaneously with monoamine oxidase inhibitors (MAOI's) (see Precautions).
Special Population Medication
Precautions for children: The safety and effectiveness of this drug for children have not been determined. Precautions for pregnancy and lactation: The safety of this product for human pregnancy has not been determined. Therefore, unless the benefits to the patient far outweigh the theoretical risks to the fetus or infant, it should not be taken during pregnancy and lactation. Precautions for the elderly: Elderly patients over 65 years old should reduce the dosage as appropriate.
Drug Interactions
This product cannot be taken with monoamine oxidase inhibitors (such as isocarboxazid and moclobemide used to treat depression, and deprenyl hydrochloride used to treat Parkinson's disease), otherwise it will cause serious side effects. The interval between the use of this product and monoamine oxidase inhibitors should be more than 14 days. However, if a reversible monoamine oxidase inhibitor with a short half-life, such as moclobemide (half-life of 1 to 2 hours), is used, it can be used one day after stopping the medication. Central nervous system drugs - Since citalopram mainly acts on the central nervous system, it should be used with caution when used in combination with other drugs that act on the central nervous system. Alcohol - Although citalopram did not enhance the effects of alcohol on sensory and motor nerves in clinical trials, like other drugs for the treatment of psychiatric disorders, it is recommended to take citalopram with a depressant.
