Isosorbide Mononitrate for Injection

Isosorbide mononitrate is the main active metabolite of isosorbide dinitrate, and most of the clinical activity of dinitrate is isosorbide mononitrate. The main pharmacology of isosorbide mononitrate and all organic nitrates is to relax vascular smooth muscle and dilate arterial terminals, especially veins. Dilation of veins can promote the aggregation of peripheral blood and reduce venous return to the heart, thereby reducing left ventricular end diastolic pressure and pulmonary capillary wedge pressure. Arteriolar dilation can reduce systemic vascular resistance, arterial systolic pressure and mean arterial pressure, and also dilate coronary arteries. The importance of reducing anterior diastolic pressure, posterior diastolic pressure and coronary dilation is not clear. Isosorbide mononitrate will not produce gene mutations or chromosomal aberrations at relevant concentrations. Male rats were started taking isosorbide mononitrate at a dose of 750 mg/kg/day 9 weeks before mating and female rats were taken 2 weeks before mating. The results showed that there was no effect on reproduction. In teratogenic studies, pregnant rats and rabbits took doses of 240 mg or 280 mg/kg/day, and no teratogenic effects were found.

Isosorbide mononitrate for injection.

This product is white or off-white loose mass or powder.

Prevent and treat angina pectoris, and use it in combination with digitalis and (or) diuretics to treat chronic heart failure.

Before intravenous drip, take 1 bottle, add 0.9% sodium chloride injection or 5% glucose injection to dissolve and dilute, then drip intravenously. The drug dosage can be adjusted according to the patient's response. The general effective dose is 2-7 mg per hour. The starting dose of intravenous drip is 60 μg/min, and the general dose is 60-120 μg/min, once a day, and a course of 10 days.

1. Common symptoms include: dizziness, vertigo, fainting, flushing of cheeks and neck caused by postural hypotension; in severe cases, persistent headache, nausea, vomiting, tachycardia, irritability; rash, blurred vision, and dry mouth are rare. 2. Clinical manifestations of overdose, in order of incidence, are: bluish lips and nails, dizziness, headache, shortness of breath, extreme fatigue, fast and weak heartbeat, fever, and even convulsions.

1. Cerebral hemorrhage or head trauma, because this product can increase intracranial pressure. 2. When patients with severe anemia use this product, it may increase the burden on the heart. 3. Patients with glaucoma are prohibited from using this product, because this product can increase intraocular pressure. 4. Myocardial infarction patients with hypotension and tachycardia. 5. Obstructive cardiomyopathy may aggravate angina pectoris. 6. Severe renal impairment. 7. Severe liver impairment may increase the risk of denatured hemoglobin.

1. When used with acetylcholine, histamine or norepinephrine, the therapeutic effect may be weakened. 2. When used with other sympathomimetic amines such as phenylephrine, ephedrine or epinephrine, the anti-anginal effect may be reduced. 3. Moderate or excessive drinking may cause low blood pressure. 4. When used with antihypertensive drugs or vasodilators, the postural hypotensive effect of nitroglycerin may be enhanced. 5. When used with tricyclic antidepressants, the hypotensive and anticholinergic effects of antidepressants may be aggravated.
