Function and Efficacy

This product is a hypoglycemic drug. 1. It stimulates the pancreatic islet beta cells to secrete insulin, which presupposes that the islet beta cells also have a certain ability to synthesize and secrete insulin; 2. It inhibits liver glycogenolysis and gluconeogenesis by increasing portal vein insulin levels or directly acting on the liver, and reduces the generation and output of glucose in the liver; 3. It may also increase the sensitivity of extrapancreatic tissues to insulin and the utilization of sugar (probably mainly through post-receptor effects). Therefore, the overall effect is to reduce fasting blood sugar and postprandial blood sugar.

Appearance

This product is white tablets.

Indication

It is suitable for mild and moderate type 2 diabetes mellitus that is not effectively controlled by diet alone. The patient's pancreatic β cells have a certain insulin secretion function and there are no serious complications.

Usage and Dosage

The usual oral dosage is 0.5g at a time, 1-2g per day. Start by taking 0.5g before breakfast or before breakfast and lunch, or 0.25g, three times a day, half an hour before meals. Gradually increase the dosage according to the condition. The general dosage is 1.5g per day, and the maximum dosage is 3g per day.

Adverse Reactions

There may be diarrhea, nausea, vomiting, headache, stomach pain or discomfort; less common is rash; rare but serious symptoms include jaundice, liver damage, bone marrow suppression, granulocytopenia (manifested as sore throat, fever, infection), thrombocytopenia (manifested as bleeding, purpura), etc.

Precautions

The following situations should not use it: 1. Patients with type I diabetes; 2. Patients with type II diabetes accompanied by stressful situations such as ketoacidosis, coma, severe burns, infection, trauma and major surgery; 3. Patients with liver and kidney dysfunction; 4. Patients allergic to sulfonamides; 5. Patients with leukopenia.

Special Population Medication

Precautions for children: Not clear yet. Precautions for pregnancy and lactation: 1. Animal experiments and clinical observations have shown that sulfonylurea hypoglycemic drugs can cause stillbirth and fetal malformation, and are contraindicated for use by pregnant women. 2. This type of drug can be excreted through breast milk, so nursing mothers should not take it to avoid hypoglycemia in infants. Precautions for the elderly: Elderly patients and those with renal insufficiency have reduced metabolism and excretion capabilities for this type of drug, so the dosage should be reduced and long-acting preparations should not be used.

Drug Interactions

1. When taken with alcohol, it can cause abdominal cramps, nausea, vomiting, headache, facial flushing and hypoglycemia. 2. When used with beta-receptor blockers, it can increase the risk of hypoglycemia and mask the symptoms of hypoglycemia, such as increased pulse rate and blood pressure. The possibility of causing this situation is low when using a small amount of selective beta-receptor blockers such as atenolol and metoprolol. 3. Chloramphenicol, guanethidine, insulin, monoamine oxidase inhibitors, phenylbutazone, hydroxyphenylbutazone, probenecid, salicylates, and sulfonamides can enhance the hypoglycemic effect when used with this product. 4. Adrenal cortical hormones, epinephrine, phenytoin sodium, thiazide diuretics, and thyroxine can increase blood sugar levels. When used with this class of drugs, the dosage of this class of drugs may need to be increased. 5. When coumarin anticoagulants are used with this class of drugs, the plasma concentrations of each other increase at first, but then the plasma concentrations of each other decrease, so the dosage of both needs to be adjusted.
