Function and Efficacy
This product is a monoquaternary ammonium steroid medium-acting non-depolarizing muscle relaxant, similar in structure to pancuronium bromide, which blocks the conduction between nerve endings and striated muscles by competing with acetylcholine for nicotinic receptors located at the motor end plate of striated muscles. Unlike depolarizing neuromuscular blocking drugs, such as succinylcholine, this product does not cause muscle fiber bundles to tremble. Intravenous injection of 0.08-0.1mg/kg takes effect within 1 minute, reaches a peak in 3-5 minutes, and lasts for 30-90 minutes. The muscle relaxant effect is 3 times stronger than that of tubocurarine chloride; it has no vagus nerve blocking effect. Since vecuronium bromide does not cause an increase in heart rate, it is suitable for patients with myocardial ischemia and heart disease, but the use of vagus nerve excitants and β-receptor blockers is prone to bradycardia. This product has a weak histamine release effect, and there are also bronchospasm and allergic reactions, but they are rare.

Indication
It is mainly used as an auxiliary drug for general anesthesia, for tracheal intubation during general anesthesia and muscle relaxation during surgery.
Usage and Dosage
This product is only for intravenous injection or intravenous drip, not for intramuscular injection. Solvent: This product can be dissolved in the following injections to a concentration of 1mg/ml for use, sterile water for injection, 5% glucose injection, 0.9% sodium chloride injection, lactated Ringer's solution, glucose sodium chloride injection; Diluent: After this product is dissolved in sterile water for injection, it can be mixed and diluted with the following injections to a concentration of 40mg/L for use, 0.9% sodium chloride injection, 5% glucose injection, Ringer's solution, glucose Ringer's solution. 1 Common dosage for adults; ① The dosage for endotracheal intubation is 0.08-0.12mg/kg, and the intubation state is reached within 3 minutes; ② When maintaining muscle relaxation at the level of neurosedative analgesia anesthesia, it is 0.05mg/kg, and when inhalation anesthesia is 0.03mg/kg. It is best to add a maintenance dose when the twitch height recovers to 25% of the control value. Infants under 21 years old are more sensitive to this product and should be tested in a small amount. The time required for muscle tension recovery is 1.5 times longer than that of adults. Especially for infants under 4 months old, the first dose is 0.01-0.02 mg/kg. If the twitch reaction is not suppressed to 90%-95%, additional doses can be given. The dose required for infants from 5 months to 1 year old is similar to that for adults, but because the effect and recovery time are longer than those for adults and children, the maintenance dose should be reduced as appropriate. Similar to adults, in pediatric patients. When the twitch degree recovers to 25% of the control value, repeat additional 1/4 of the initial dose as a maintenance medication, and no accumulation effect will occur. 3 Reduce the dosage for obese patients; Caesarean section and neonatal surgery should not exceed 0.1 mg/kg.
Adverse Reactions
1. Allergic reactions: ①. Allergic reactions to neuromuscular blocking drugs have been reported. Although this product is rare, it should be noted; ②. Cross-allergic reactions may occur between neuromuscular blocking drugs, so people with a history of allergies should be particularly cautious when using vecuronium. 2. Histamine release and histamine-like reactions: Local or systemic histamine-like reactions may occur occasionally in clinical practice.
Precautions
Patients with a history of allergy to vecuronium or bromide ion are contraindicated.
Special Population Medication
Precautions for children: Due to the different sensitivity of children's neuromuscular junctions, especially for newborns (less than four weeks) and infants (less than four months), the first dose is 0.01-0.02 mg/kg. If the twitch reaction is not suppressed to 90-95%, additional doses can be given. In clinical surgery, the dosage should not exceed 0.1 mg/kg. The dosage required for infants and young children aged 5 months to 1 year is similar to that for adults. However, the onset time of this product in newborns and infants is shorter than that in adults and children. Therefore, it is not necessary to increase the intubation dose if you want to shorten the intubation time. Compared with adults and children, this product has a longer effect and recovery time in newborns and infants, and the maintenance dose should be reduced as appropriate. Application in pediatrics: - The ED90 of newborns and infants under halothane anesthesia is higher than that of adults (≈28 μg/kg), but there is no statistically significant difference. Compared with adults, its effect and recovery time are reduced by 30% and 20-30%, respectively. Similar to adults, in pediatric patients, when the height of the twitch recovers to 25% of the control value, repeat the initial dose of 1/4 as maintenance medication. Repeated maintenance doses of this product will not cause accumulation. Although the recovery time is longer in newborns and infants, antagonists are not required to be used routinely. If antagonists are used, they will be as effective as in adults. Precautions during pregnancy and lactation: There is currently insufficient data to prove that the use of this product in animals or humans during pregnancy has potential harm to the fetus, but it can only be used by pregnant women after the attending physician has weighed the pros and cons. It is not clear whether this product is contained in breast milk. Caesarean section: Studies have shown that it is safe to use this product in caesarean sections, not more than 0.1 mg/kg. This product does not affect Apgar scores, fetal muscle tension, and the regulatory ability of the heart and lungs. Analysis of umbilical cord blood samples confirmed that only a very small amount of this product passed the placenta and had no adverse reactions to the fetus. Note: Patients using magnesium sulfate preparations for pregnancy toxemia have unsatisfactory recovery of neuromuscular blockade when using this product. This is because magnesium salts can enhance the neuromuscular blocking effect. Therefore, for patients receiving magnesium sulfate treatment, the dosage of this product should be reduced, and it should be carefully dripped according to the twitch reaction. Precautions for the elderly: If you suffer from cardiovascular disease, advanced age, edema, etc., which will lead to an increase in distribution volume, it may prolong the onset time.
Drug Interactions
1 The following drugs can enhance the effect of this product: ① Inhaled anesthetics such as halothane, enflurane, isoflurane, etc.; ② High-dose thiopental sodium, methylthiobarbital, ketamine, fentanyl, γ-hydroxybutyric acid, etomidate, propofol; ③ Other non-depolarizing muscle relaxants and succinylcholine; ④ Antibiotics such as aminoglycosides, peptides, acylaminopenicillins and high-dose metronidazole; ⑤ Others such as diuretics, β-adrenergic blockers, thiamine, monoamine oxidase inhibitors, quinidine, protamine, α-adrenergic blockers, magnesium salts, etc. 2 The following drugs can weaken the effect of this product: ① Neostigmine, edrophonium, pyridostigmine, aminopyridine derivatives; ② Long-term use of corticosteroids or amide zosin; ③ Norepinephrine, azathioprine (only short-term, limited effect), theophylline, calcium chloride. 3 The following drugs may change the effect of this product: after using vecuronium bromide, giving depolarizing muscle relaxants, such as succinylcholine, may enhance or weaken its neuromuscular blocking effect.
