Function and Efficacy

This product is an amide local anesthetic. After blood absorption or intravenous administration, it has obvious biphasic effects of excitation and inhibition on the central nervous system, and there may be no precursor excitation. When the blood concentration is low, analgesia and drowsiness occur, and the pain threshold increases; with increasing doses, the effect or toxicity increases, and it has an anticonvulsant effect at sub-toxic blood concentrations; convulsions may occur when the blood concentration exceeds 5μg·ml-1. At low doses, this product can promote the outflow of K in myocardial cells, reduce myocardial autonomy, and have an anti-ventricular arrhythmia effect; at therapeutic doses, it has no obvious effect on the electrical activity of myocardial cells, atrioventricular conduction, and myocardial contraction; further increases in blood concentrations can cause a slowing of heart conduction velocity, atrioventricular conduction block, inhibition of myocardial contractility, and a decrease in cardiac output.

Ingredients

Appearance

This product is a colorless clear liquid.

Indication

This product is a local anesthetic and antiarrhythmic drug. It is mainly used for infiltration anesthesia, epidural anesthesia, surface anesthesia (including mucosal anesthesia during thoracoscopy or abdominal surgery) and nerve conduction block. This product can also be used for ventricular premature beats and ventricular tachycardia after acute myocardial infarction, and can also be used for ventricular arrhythmias caused by digitalis poisoning, cardiac surgery and cardiac catheterization. This product is usually ineffective for supraventricular arrhythmias.

Usage and Dosage

1. For anesthesia (1) Common dosage for adults: ① Surface anesthesia: 2% to 4% solution, not more than 100 mg at a time. When injecting, the single dose should not exceed 4.5 mg/kg (without epinephrine) or 7 mg/kg per dose (with epinephrine at a concentration of 1:200000). ② Sacral block for labor analgesia: use 1.0% solution, limited to 200 mg. ③ Epidural block: 1.5% to 2.0% solution for thoracolumbar segment, 250 to 300 mg. ④ Infiltration anesthesia or intravenous regional block: use 0.25% to 0.5% solution, 50 to 300 mg.

Adverse Reactions

1. This product can act on the central nervous system, causing adverse reactions such as drowsiness, paresthesia, muscle tremor, convulsion, coma and respiratory depression. 2. It can cause hypotension and bradycardia. If the blood concentration is too high, it can cause slowing of atrial conduction velocity, atrioventricular conduction block, inhibition of myocardial contractility and decreased cardiac output.

Precautions

1. Patients who are allergic to local anesthetics are contraindicated. 2. Patients with Adams syndrome (acute cardiogenic cerebral ischemic syndrome), preexcitation syndrome, and severe heart block (including sinoatrial, atrioventricular and intraventricular conduction block) are contraindicated for intravenous administration.

Drug Interactions

1 When used in combination with cimetidine and beta-blockers such as propranolol, metoprolol, and nadolol, lidocaine metabolism in the liver is inhibited, lidocaine blood concentration increases, and adverse reactions of the heart and nervous system may occur. The lidocaine dose should be adjusted, and electrocardiogram monitoring and lidocaine blood concentration should be performed. 2 Barbiturates can promote lidocaine metabolism, and the combination of the two drugs can cause bradycardia and sinus arrest. 3 When used in combination with procainamide, transient delirium and hallucinations may occur, but it does not affect the blood concentration of this product. 4 Isoproterenol increases the total clearance of this product due to increased hepatic blood flow; norepinephrine reduces the total clearance of this product due to reduced hepatic blood flow.
