Caffeine is a central nervous system stimulant that can excite the cerebral cortex, increase sensitivity to the outside world, and constrict cerebral blood vessels, enhancing the effect of the first two drugs in relieving headaches. Indication
Used for fever, headache, neuralgia, toothache, menstrual pain, muscle pain, and joint pain.
Usage and Dosage
Oral administration, Adults: 1-2 tablets at a time, 3 times a day after meals.
Adverse Reactions
This product is a compound tablet mainly composed of aspirin and phenacetin. The more common adverse reactions of aspirin include gastrointestinal reactions such as nausea, vomiting, upper abdominal discomfort or pain, which usually disappear after stopping the drug; gastrointestinal bleeding or ulcers may occur when used for a long time or in large quantities; reversible tinnitus and hearing loss may occur after taking a certain course of treatment; a few patients may develop allergic reactions such as asthma, urticaria, angioedema or shock, which may lead to death in severe cases; excessive doses may cause liver and kidney damage. Phenacetin can cause renal papillary necrosis, interstitial nephritis and acute renal failure, and may even induce renal pelvic cancer and bladder cancer. Phenacetin is also easy to use hemoglobin to form methemoglobin, which reduces the oxygen-carrying capacity of the blood and causes cyanotic reactions. In addition, phenacetin can also cause hemolysis and hemolytic anemia, and has certain toxicity to the retina. Long-term use of phenacetin can also cause drug dependence. Phenacetin can also cause liver damage.
Precautions
It is contraindicated for patients who are allergic to aspirin or other nonsteroidal anti-inflammatory drugs and caffeine, hemophilia, active peptic ulcer and other gastrointestinal bleeding. It is contraindicated for infants under 3 months old.
Special Population Medication
Precautions for children: Children, especially those with fever and dehydration, are prone to toxic reactions; children with acute febrile diseases, especially influenza and chickenpox, may develop Reye syndrome if this product is used. Children under 6 years old should be used with caution, and infants under 3 months old are prohibited. Precautions for pregnancy and lactation: Aspirin in this product easily crosses the placenta. Considering the possible impact on the fetus, it is not suitable for pregnant women. After lactating women take this product, aspirin can reach a certain concentration in breast milk, so long-term high-dose use may cause adverse reactions such as bleeding tendency in infants. Precautions for the elderly: Elderly patients are prone to toxic reactions due to decreased renal function when taking this product, so it should be used with caution or in an appropriate reduced dose.
Drug Interactions
1. The efficacy is not enhanced when used with other non-steroidal anti-inflammatory analgesics, because this product can reduce the bioavailability of other non-steroidal anti-inflammatory drugs. In addition, gastrointestinal side effects (including ulcers and bleeding) are increased; in addition, due to the enhanced inhibitory effect on platelet aggregation, the risk of bleeding in other parts can also be increased. Long-term and large-scale use of this product with acetaminophen may cause kidney diseases including renal papillary necrosis, renal cancer or bladder cancer; 2. When used with any drug that can cause hypoprothrombinemia, thrombocytopenia, reduced platelet aggregation function or gastrointestinal ulcer bleeding, there may be a risk of aggravating coagulation disorders and causing bleeding; 3. Used with anticoagulants (dicoumarol, heparin, etc.) and thrombolytics (streptokinase, urokinase) may increase the risk of bleeding; 4. Urine alkalinizing drugs (sodium bicarbonate, etc.) and antacids (long-term and large-scale use) can increase the excretion of this product in urine, causing a decrease in blood drug concentration. However, when the blood concentration of this product has reached a stable state and the alkaline drug is discontinued, the blood concentration of this product may rise to a toxic level. Carbonic anhydrase inhibitors can alkalinize urine, but can cause metabolic acidosis, which can not only reduce the blood concentration, but also increase the amount of this product that penetrates into the brain tissue, thereby increasing toxic reactions; 5 Uric acid drugs can reduce the excretion of this product and increase its blood concentration. Patients whose blood concentration of this product has reached a stable state may increase the blood concentration of this product and increase toxic reactions after adding uricidifying drugs. 6 Glucocorticoids (hormones for short) can increase the excretion of salicylates. In order to maintain the blood concentration of this product when used at the same time, the dosage of this product should be increased if necessary. Long-term use of this product with hormones, especially when used in large quantities, increases the risk of gastrointestinal ulcers and bleeding. For this reason, it is not recommended to use these two drugs at the same time in clinical practice; 7 The hypoglycemic effect of insulin or oral hypoglycemic drugs can be enhanced and accelerated by using this product together; 8 When used together with methotrexate (MTX), it can reduce the binding of methotrexate to proteins, reduce its excretion from the kidneys, increase blood drug concentrations and increase toxic reactions; 9 The uric acid excretion effect of probenecid or sulfinpyrazone can be reduced by using this product at the same time; when the blood concentration of salicylate is 50mu;g/ml, it is significantly reduced, and it is even worse when it is 100-150mu;g/ml. In addition, probenecid can reduce the clearance rate of salicylate from the kidneys, thereby increasing the latter's blood concentration.
