Nitroglycerin Injection

The main pharmacological action is to relax vascular smooth muscle. Nitroglycerin releases nitric oxide (NO), which is the same as endothelial relaxing factor, activating guanylate cyclase, increasing cyclic guanosine monophosphate (cGMP) in smooth muscle and other tissues, leading to dephosphorylation of myosin light chain, regulating the contraction state of smooth muscle, and causing vasodilation. Nitroglycerin dilates the arteriovenous vascular bed, mainly dilating the veins, and its intensity is dose-related. Peripheral vein dilation causes blood to remain in the periphery, reducing the amount of blood returning to the heart and reducing the left ventricular end-diastolic pressure (preload). Dilation of arteries reduces peripheral resistance (afterload). Arteriovenous dilation reduces myocardial oxygen consumption and relieves angina pectoris. It also has a dilating effect on epicardial coronary artery branches. Therapeutic doses can reduce systolic blood pressure, diastolic blood pressure and mean arterial pressure, and effective coronary perfusion pressure can often be maintained, but when blood pressure is excessively reduced or heart rate increases, shortening the diastolic filling time, effective coronary perfusion pressure is reduced. It reduces the increased central venous pressure and pulmonary capillary wedge pressure, pulmonary vascular resistance and systemic vascular resistance. The heart rate usually increases slightly, which is probably a reflex effect of the drop in blood pressure. The cardiac index may increase, decrease or remain unchanged. In patients with increased left ventricular filling pressure and peripheral resistance and low cardiac index, the cardiac index may increase. On the contrary, in patients with normal left ventricular filling pressure and cardiac index, intravenous injection of the drug can slightly reduce the cardiac index. There are no long-term animal studies to evaluate its carcinogenicity.

Nitroglycerin.

This product is a colorless clear liquid.

It is used for the treatment and prevention of angina pectoris caused by coronary heart disease, and can also be used to lower blood pressure or treat congestive heart failure.

Injection: Dilute with 5% glucose injection or sodium chloride injection and drip intravenously. The initial dose is 5μg/min. It is best to use an infusion pump to infuse at a constant rate. For lowering blood pressure or treating heart failure, increase by 5μg/min every 3 to 5 minutes. If it is ineffective at 20μg/min, increase by 10μg/min, and then 20μg/min. There are great individual differences in the response of patients to this drug. There is no fixed suitable dose for intravenous drip. The dosage should be adjusted according to the individual's blood pressure, heart rate and other hemodynamic parameters.

1. Headache: It may occur immediately after taking the medicine and may be severe and persistent. 2. Dizziness, weakness, palpitations and other manifestations of postural hypotension may occasionally occur, especially in patients who are standing or immobilized. 3. Significant hypotension reactions may occur at therapeutic doses, manifested as nausea, vomiting, weakness, sweating, pallor and collapse. 4. Syncope, flushing, drug rash and exfoliative dermatitis have all been reported.

It is contraindicated in patients with early myocardial infarction (with severe hypotension and tachycardia), severe anemia, glaucoma, increased intracranial pressure, and known allergy to nitroglycerin. It is also contraindicated in patients using sildenafil citrate (Viagra), which enhances the antihypertensive effect of nitroglycerin.

Precautions for children: The safety and efficacy of the drug for children are still uncertain. Precautions for pregnancy and lactation: It is not known whether it will cause fetal harm or affect fertility, so it should only be used in pregnant women when it is necessary. It is also not known whether it is excreted from human breast milk, so pregnant women should be cautious when using it intravenously. Precautions for the elderly: It is not yet clear. Combination with antihypertensive drugs or vasodilators may enhance the orthostatic hypotensive effect of nitrates; 3. Aspirin may reduce the clearance of sublingual nitroglycerin and enhance its hemodynamic effects; 4. The use of long-acting nitrates may reduce the therapeutic effect of sublingual medications; 5. Sildenafil citrate (Viagra) enhances the antihypertensive effect of organic nitrates; 6. The efficacy may be weakened when used in combination with acetylcholine, histamine and sympathomimetic amines.
