Function and Efficacy
This product is the main isomer of doxorubicin, which is the trans configuration of the C4 hydroxyl group in the amino sugar part of doxorubicin. It can be directly embedded in DNA, form a complex with the double helix structure of DNA, block the formation of RNA dependent on DNA, and has the kinetic energy to form superoxide radicals. The site of action of this product is on the C4 hydroxyl group of the amino sugar part (its excretion amount can reach 1/3 of the total excretion amount). This was only found after using this drug, but not when using daunorubicin and doxorubicin. This may be the main reason why epirubicin is cleared faster in the body and its toxicity is lower than that of doxorubicin at the same dose.

Appearance
This product is bright red or orange-red loose lumps; it is hygroscopic.
Indication
It is mainly used for various types of acute leukemia and malignant lymphoma, breast cancer, bronchial lung cancer, ovarian cancer, Wilms' tumor, soft tissue sarcoma, bladder cancer, testicular cancer, prostate cancer, gastric cancer, liver cancer (including primary hepatocellular carcinoma and metastatic cancer) and medullary thyroid cancer and other solid tumors. It is mainly used for various types of acute leukemia, malignant lymphoma (including Hodgkin's lymphoma), breast cancer, bronchial lung cancer, ovarian cancer, Wilms' tumor, soft tissue sarcoma, bladder cancer, testicular cancer, gastric cancer, medullary thyroid cancer and prostate cancer, and is also suitable for hepatic artery catheterization chemotherapy of primary and metastatic liver cancer. It is particularly effective for leukemia, lymphoma and breast cancer, and is also effective for ovarian cancer.
Usage and Dosage
When epirubicin is used alone, the adult dose is 60-90 mg/m2 per body surface area. When combined with chemotherapy, 50-60 mg/m2 is injected intravenously each time. It can be repeated every 21 days depending on the patient's blood picture.
Adverse Reactions
1. Common symptoms include hair loss (approximately 70; 80% of patients), bone marrow suppression (approximately 50; 60%, white blood cells may drop to the lowest point 10-14 days after medication, and gradually recover in about 3 weeks, anemia and obvious thrombocytopenia are rare), loss of appetite, nausea, and vomiting, but compared with doxorubicin at equivalent doses, the degree is milder than doxorubicin. 2. Myocardial toxicity is also milder than doxorubicin, and its incidence and severity are proportional to the cumulative amount of this product. Although abnormal heart rhythm and tachycardia are common after medication, they are mostly transient and recover quickly; delayed severe ejection heart failure mostly occurs half a year after medication or when the total dose exceeds 700; 800 mg. It should be noted that this severe myocardial damage can sometimes occur suddenly
Precautions
1) This product has potential teratogenic, mutagenic and carcinogenic effects in animals, but there is a lack of clear evidence in humans. 2) Epirubicin, like daunorubicin and doxorubicin, can cross the placenta. Therefore, this product is contraindicated within the first 3 months of pregnancy and should not be used by breastfeeding women. 3) A certain amount of epirubicin is excreted through the kidneys after use. The dose should be reduced for elderly patients or those with significantly reduced renal function. 4) Epirubicin is contraindicated in the following situations: those who have used sufficient doses of daunorubicin or doxorubicin (total dose ge; 400-500 mg/m2) in the past or have an allergic reaction to these two drugs, peripheral blood white blood cells less than 3500 or platelets less than 50,000, fever or severe infection, cachexia, dehydration, electrolyte or acid-base imbalance, gastrointestinal
Drug Interactions
1 Epirubicin can be used in combination with other anti-tumor drugs, but the dosage of epirubicin should be reduced. When used in combination, they should not be used in the same syringe. 2 Epirubicin should not be mixed with heparin for injection, because the two are chemically incompatible and precipitation reactions will occur at a certain concentration.
