Acyclovir for injection
Function and Efficacy
Antiviral drug. It has an inhibitory effect on herpes simplex virus, varicella zoster virus, cytomegalovirus, etc. in vitro. After entering the cells infected by herpes virus, this product competes with deoxynucleoside for viral thymidine kinase or cell kinase, and the drug is phosphorylated into activated acyclovir triphosphate, and then inhibits viral replication in two ways: ① Interfering with viral DNA polymerase to inhibit viral replication; ② Under the action of DNA polymerase, it binds to the growing DNA chain, causing the extension of the DNA chain to be interrupted. This product has a special affinity for viruses, but has low toxicity to mammalian host cells. There are reports of carcinogenicity in in vitro cell transformation assays, but no evidence of carcinogenicity has been found in animal experiments. Some animal experiments show that high concentrations of drugs can cause mutations, but there is no evidence of chromosomal changes. The carcinogenic and mutagenic effects of this product are still unclear. High-dose injection can cause testicular atrophy and reduced sperm count in animals. The drug can pass through the placenta, and animal experiments have confirmed that it has no effect on the embryo.

Indication
Infections caused by herpes simplex virus and varicella-zoster virus, as well as herpes simplex, varicella and herpes zoster in immunocompromised patients.
Usage and Dosage
Generally, a single dose of 5 mg/kg is intravenously dripped 3 times a day, dripped once every eight hours, and each drip time is more than 1 hour, and the drug is given continuously for 7 days. Preparation of injection: Dissolve each bottle (containing acyclovir 0.25) with 10 ml of injection water or normal saline, then add 100 ml of balance solution to dilute each bottle, and prepare it temporarily before use.
Adverse Reactions
1 Allergic reaction: Sometimes allergic symptoms such as rash, urticaria, fever, etc. occur, which disappear after stopping the drug. 2 Nervous system: Rarely, there are convulsions of lower limbs, numbness of tongue and hands and feet, tremor, and general fatigue. In this case, the dosage can be reduced or the drug can be discontinued, and appropriate treatment can be given. 3 Kidney: Sometimes, there are increases in blood urea nitrogen and serum creatinine values, proteinuria, and red blood cells and mucus in urine sediment.
Precautions
People who are allergic to this product are prohibited from using it. People with abnormal renal function, children and pregnant women should use it with caution.
Drug Interactions
1. When used intravenously with interferon or methotrexate (intrathecal), it may cause mental abnormalities and should be used with caution. 2. When used intravenously with nephrotoxic drugs, it can aggravate nephrotoxicity, especially in patients with renal insufficiency. 3. When used in combination with zidovudine, it can cause nephrotoxicity, manifested as deep lethargy and fatigue. 4. It competitively inhibits the secretion of organic acids with probenecid. The combined use of probenecid can slow down the excretion of this product, prolong the half-life, and cause drug accumulation in the body.
