Function and Efficacy
According to literature reports, this product is a hypoglycemic drug. This product can reduce fasting and postprandial hyperglycemia in patients with type 2 diabetes, and HbAlc can be reduced by 1% to 2%. The mechanism of this product in lowering blood sugar may be: 1. Increase the sensitivity of peripheral tissues to insulin and increase insulin-mediated glucose utilization. 2. Increase the utilization of glucose by non-insulin-dependent tissues, such as the brain, blood cells, renal medulla, intestines, skin, etc. 3. Inhibit hepatic gluconeogenesis and reduce hepatic glucose output. 4. Inhibit intestinal wall cells from taking up glucose. 5. Inhibit the biosynthesis and storage of cholesterol and reduce blood triglyceride and total cholesterol levels. Unlike the effect of insulin, this product has no effect of promoting fat synthesis and no obvious hypoglycemic effect on normal people. It generally does not cause hypoglycemia when used alone for type 2 diabetes.

Appearance
This product is a sugar-coated or film-coated tablet, which appears white after removing the coating.
Indication
It is used for patients with type 2 diabetes who are not satisfied with diet control alone, especially those who are obese and have hyperinsulinemia. This drug not only has the effect of lowering blood sugar, but also may reduce weight and hyperinsulinemia. It can be effective for patients with poor efficacy of certain sulfonylureas. For example, it is better to use it in combination with sulfonylureas, intestinal glycosidase inhibitors or thiazolidinediones hypoglycemic drugs than to use them alone.
Usage and Dosage
1. Take the medicine as directed by the doctor. This product should be started at a small dose and the dose should be gradually increased according to the patient's condition. 2. Usually the starting dose of this product is 0.5 grams twice a day; or 0.85 grams once a day; taken with meals. 3. It can be increased by 0.5 grams per week, or 0.85 grams every 2 weeks, gradually increasing to 2 grams per day, taken in divided doses. 4. The maximum recommended dose for adults is 2550 mg per day. For patients who need to further control blood sugar, the dose can be increased to 2550 mg per day (that is, 0.85 grams each time, three times a day). 5. When the daily dose exceeds 2 grams, in order to better tolerate, the drug is best taken in divided doses with three meals.
Adverse Reactions
1. Common symptoms include: nausea, vomiting, diarrhea, metallic taste in the mouth. 2. Sometimes there is weakness, tiredness, dizziness, and rash. 3. Although the incidence of lactic acidosis is very low, it should be paid attention to. Clinical manifestations include vomiting, abdominal pain, hyperventilation, mental disorder, and increased lactic acid concentration in the blood that cannot be explained by uremia, ketoacidosis or salicylic acid poisoning. 4. It can reduce the intestinal absorption of vitamin B12, reduce hemoglobin, produce megaloblastic anemia, and also cause malabsorption.
Precautions
1. Type II diabetes with ketoacidosis, liver and kidney dysfunction (serum creatinine exceeds 1.5mg/dl), pulmonary dysfunction, heart failure, acute myocardial infarction, severe infection and trauma, major surgery, and clinical hypotension and hypoxia. 2. Diabetes with severe chronic complications (such as diabetic nephropathy, diabetic fundus lesions). 3. Before intravenous pyelography or arterial angiography. 4. Alcoholics. 5. Patients with severe heart and lung diseases. 6. Patients with vitamin B12, folic acid and iron deficiency. 7. Patients with poor systemic condition (such as malnutrition, dehydration).
Special Population Medication
Precautions for children: The maximum daily dose of this product for patients with type 2 diabetes aged 10-16 years is 2000 mg. This product is not recommended for children under 10 years old. Precautions for pregnancy and lactation: This product is not recommended for pregnant women. Lactating women should use this product with caution. If this product must be used, breastfeeding should be stopped. Precautions for the elderly: Elderly patients over 65 years old should use this product with caution and have their renal function checked regularly. The maximum dose is usually not used. This product is not recommended for patients over 80 years old unless their creatinine clearance test shows that their renal function is not reduced.
Drug Interactions
1. The combined use of metformin and glibenclamide as a single dose did not change the pharmacokinetic parameters of metformin. 2. When metformin was used in combination with furosemide (furosemide), the AUC of metformin increased, but the renal clearance did not change; at the same time, the Cmax and AUC of furosemide decreased, the terminal half-life was shortened, and the renal clearance did not change. 3. Cationic drugs secreted by the renal tubules (such as amiloride, digoxin, morphine, procainamide, quinidine, quinine, ranitidine, triamterene, trimethoprim and vancomycin) may theoretically compete with metformin for the renal tubular transport system and interact with each other. Therefore, it is recommended to closely monitor and adjust the dose of this product and/or interacting drugs. 4. When metformin was used in combination with cimetidine, the plasma and whole blood AUC of metformin increased, but the elimination half-life of metformin did not change when the two drugs were used in combination as a single dose. The pharmacokinetics of cimetidine did not change. 5. If you are taking certain drugs that can cause blood sugar to rise at the same time, such as thiazides or other diuretics, glucocorticoids, phenothiazines, thyroid preparations, estrogens, oral contraceptives, phenytoin, nicotinic acid, sympathomimetics, ion channel blockers and isoniazid, you should monitor blood sugar closely, and pay close attention to the occurrence of hypoglycemia after these drugs are discontinued. 6. Metformin does not bind to plasma proteins, so drugs that are highly bound to proteins, such as salicylates, sulfonamides, chloramphenicol, probenecid, etc., are less likely to interact with sulfonylurea drugs, which are mainly bound to serum proteins. 7. Except for chlorpropamide, patients usually do not need a conversion period when switching from other oral hypoglycemic drugs to this product. Patients taking chlorpropamide should pay close attention to this product in the first 2 weeks of switching, because chlorpropamide has a long retention in the body, which can easily lead to excessive drug effects and hypoglycemia. 8. Metformin has an anticoagulant tendency to increase warfarin. 9. Taking resin drugs together with this product can reduce the absorption of metformin.
