Function and Efficacy
The main pharmacological action is to relax vascular smooth muscle. Nitroglycerin releases nitric oxide (NO), which is the same as endothelial relaxing factor, activating guanylate cyclase, increasing cyclic guanosine monophosphate (cGMP) in smooth muscle and other tissues, leading to dephosphorylation of myosin light chain, regulating the contractile state of smooth muscle, and causing vasodilation.

Indication
It is used for the treatment and prevention of angina pectoris caused by coronary heart disease, and can also be used to lower blood pressure or treat congestive heart failure.
Usage and Dosage
Tablets: Adults take 0.25-0.5 mg (1 tablet) sublingually at a time. Repeat 1 tablet every 5 minutes until the pain is relieved. If the pain persists after taking 3 tablets in 15 minutes, seek medical attention immediately. Use it prophylactically 5-10 minutes before activities or bowel movements to avoid inducing angina pectoris.
Adverse Reactions
1. Headache: may occur immediately after taking the drug, may be severe and persistent. 2. Dizziness, weakness, palpitations and other manifestations of postural hypotension may occur occasionally, especially in patients who are standing upright or immobilized. 3. Significant hypotension reactions may occur at therapeutic doses, manifested as nausea, vomiting, weakness, sweating, pallor and collapse. 4. Syncope, flushing, drug rash and exfoliative dermatitis have all been reported.
Precautions
It is contraindicated in patients with early myocardial infarction (with severe hypotension and tachycardia), severe anemia, glaucoma, increased intracranial pressure, and known allergy to nitroglycerin. It is also contraindicated in patients using sildenafil citrate (Viagra), which enhances the antihypertensive effect of nitroglycerin.

Precautions for children: Precautions for pregnancy and lactation: It is not known whether it will cause fetal harm or affect fertility, so it should only be used in pregnant women when necessary. It is also not known whether it is excreted in human breast milk, so lactating women should be cautious. Precautions for the elderly:
Drug Interactions
1. When drinking moderately or excessively, the use of this drug may cause hypotension. 2. When used in combination with antihypertensive drugs or vasodilators, the postural hypotensive effect of nitrates may be enhanced. 3. Aspirin may reduce the clearance of sublingual nitroglycerin and enhance its hemodynamic effects. 4. The use of long-acting nitrates may reduce the therapeutic effect of sublingual medications. 5. Sildenafil citrate (Viagra) enhances the antihypertensive effect of organic nitrates. 6. When used in combination with acetylcholine, histamine and sympathomimetic amines, the efficacy may be weakened.
