Function and Efficacy

This drug belongs to Class 2A, i.e., beta 1-receptor blockers without partial agonist activity (cardioselective beta 1-receptor blockers). It has a selective blocking effect on beta 1-receptors, no PAA (partial agonist activity), and no membrane stabilizing effect. Its beta 1-receptor blocking effect is about the same as that of propranolol (PP), and its selectivity for beta 1-receptors is slightly inferior to that of atenolol. Metoprolol's effects on the heart, such as slowing heart rate, inhibiting cardiac contractility, reducing automaticity, and delaying atrioventricular conduction time, are similar to those of propranolol and atenolol (AT). Its effects on reducing elevated blood pressure and heart rate during exercise tests are also similar to those of PP and AT. Its contraction effect on vascular and bronchial smooth muscle is weaker than that of PP, so its effect on the respiratory tract is also smaller, but still stronger than that of AT. Metoprolol can also reduce plasma renin activity. Toxicology: This product has no mutagenic effect; it has no effect on the fetus. Rats took 800 mg/day of this product for a total of 2 years and no benign or malignant vegetation was found.

Indication

It is clinically used to treat various types of hypertension (can be used in combination with diuretics and vasodilators) and angina pectoris. Intravenous injection of this product is also effective for arrhythmias, especially supraventricular arrhythmias.

Usage and Dosage

For the treatment of hypertension, 100-200 mg/time, twice a day, should be used immediately after hemodynamic stabilization. Acute myocardial infarction advocates early use, i.e., within the first few hours, because immediate use can reduce the infarction range and reduce the short-term (15-day) mortality rate in patients who have not undergone thrombolysis (this effect appears 24 hours after medication). It can reduce the reinfarction rate and re-ischemia rate in patients who have undergone thrombolysis, and can also reduce the mortality rate if the medication is used within 2 hours. General usage: Metoprolol can be injected intravenously at 2.5-5 mg/time (within 2 minutes), once every 5 minutes, for a total of 3 times of 10-15 mg. After 15 minutes, start taking 25-50 mg orally, once every 6-12 hours, for a total of 24-48 hours, and then take 50-100 mg orally, twice a day.

Adverse Reactions

1 Cardiovascular system: slowed heart rate, conduction block, lowered blood pressure, worsening heart failure, cold limbs or untouchable pulse due to peripheral vascular spasm, Raynaud's phenomenon. 2 Because of its fat solubility and easy penetration into the central nervous system, this system has more adverse reactions. Fatigue and dizziness account for 10%, depression accounts for 5%, and others include headache, dreaminess, insomnia, etc. Hallucinations are occasionally seen. 3 Digestive system: nausea, stomachache, constipation account for 1%, diarrhea accounts for 5%, but it is not serious and rarely affects medication. 4 Others: shortness of breath, joint pain, itching, retroperitoneal fibrosis, deafness, eye pain, etc.

Precautions

Hypotension, significant bradycardia (heart rate 45/minute), cardiogenic shock, severe or acute heart failure, poor peripheral circulation perfusion, second-degree or third-degree atrioventricular block, sick sinus syndrome, severe peripheral vascular disease.

Drug Interactions

Combination with cimetidine or prior use of quinidine can increase the plasma concentration of metoprolol; combination with reserpine can enhance the effect of this product, but attention should be paid to hypotension and tachycardia.
