Function and Efficacy
1Quinine is a quinoline derivative that can bind to the DNA of malarial parasites to form a complex that inhibits DNA replication and RNA transcription, thereby inhibiting the protein synthesis of the protozoa. Its effect is weaker than that of chloroquine. In addition, quinine can reduce the oxygen consumption of malarial parasites, inhibit the phosphorylase in malarial parasites and interfere with their sugar metabolism. Quinine also causes malarial pigment aggregation, but it develops slowly, rarely forms large clumps, and is often accompanied by cell death. Electron microscopy observation shows that the nucleus and outer membrane of the protozoa are swollen, and there are small vacuoles. The blood cell particles aggregate in the small vacuoles, which is different from the pigment aggregation of chloroquine. In the blood, a certain concentration of quinine can cause premature rupture of parasitized red blood cells, thereby preventing the maturation of schizonts. This product is ineffective in the early stage of malaria, and a long course of treatment can cure malignant malaria, but it has no direct effect on the gametocytes of malignant malaria, so it cannot interrupt transmission; 2Caffeine: A small dose acts on the high center of the cerebral cortex to promote mental excitement. Increasing the dose has the effect of stimulating the respiratory center and vasomotor center of the medulla oblongata.

Usage and Dosage
Deep intramuscular injection, 2 ml at a time.
Adverse Reactions
Intramuscular injection can cause tissue necrosis. If necessary, deep gluteal muscle injection should be used. Cinchona reactions may also occur in hypersensitive people, so it is necessary to pay attention. A few people with special characteristics may have acute hemolytic reactions.
Precautions
1. It is forbidden for patients with optic neuritis, tinnitus, myasthenia gravis, and cardiomyopathy; 2. It is forbidden for pregnant women.

Drug Interactions
1. Antacids and aluminum-containing preparations can delay or reduce the absorption of quinine; 2. When anticoagulants are used in combination with quinine, the anticoagulant effect can be enhanced; 3. When muscle relaxants such as succinylcholine and tubocurarine are used in combination with quinine, respiratory depression may occur; 4. When quinidine is used in combination with quinine, the cinchona reaction may increase; 5. Urine alkalinizing agents such as sodium bicarbonate may increase the reabsorption of quinine by the renal tubules, leading to an increase in quinine blood concentration and toxicity; 6. When used in combination with vitamin K, the absorption of quinine may be increased; 7. When used in combination with buclizine, cyclizine, meclizine, phenothiazines, thioxanthenes, trimethofenzamide, and aminoglycoside antibiotics, tinnitus and dizziness may occur; 8. When used in combination with nifedipine (nifedipine), the concentration of free quinine increases; 9. Caffeine has a mutually reinforcing effect with ephedrine and adrenaline, so they should not be injected at the same time.
