Physostigmine salicylate eye ointment

Function and Efficacy

This product can reversibly inhibit cholinesterase, so that acetylcholine released by cholinergic nerve endings is protected from hydrolysis by this enzyme, thus showing the muscarinic and nicotinic effects of acetylcholine. Local eye drops can shrink the pupil, reduce intraocular pressure, contract the ciliary muscle and cause accommodation spasm, etc. The effect is stronger and more lasting than pilocarpine, but it is more irritating. Because of the strong effect of contracting the ciliary muscle, it can cause eye pain and headache. This product loses its original effect on tissues de-innervated by parasympathetic nerves.

Ingredients

The main ingredient is salicylic acid physostigmine. Its chemical name is 1,2,3,3a-hexahydro-1,3a,8-trimethylpyrrolo[2,3-b]indol-5-phenol methylcarbamate salicylate. Molecular formula: C15H21N3O2C7H6O3, molecular weight: 413.47.

Appearance

This product is a light yellow or yellow ointment.

Indication

Primary angle-closure glaucoma, occasionally used for primary open-angle glaucoma.

Usage and Dosage

Apply to the inner eyelids before going to bed at night. Generally, use pilocarpine during the day and this product at night. Or follow the doctor's advice.

Adverse Reactions

This product can cause eyelid spasm after eye drops. The drug is absorbed through the skin, causing inactivation of cholinesterase in the eyelid and accommodation spasm. This adverse reaction is more common in young people and children. Ciliary muscle spasm headache, eye pain, and ciliary congestion are very common. In addition, there is accommodation spasm myopia. Contact allergic dermatitis is occasionally seen with long-term medication. It is extremely rare for this product to cause systemic toxic reactions after eye drops. Occasionally, systemic toxic reactions such as salivation, tearing, sweating, nausea, vomiting, abdominal pain, bronchiolar spasm, and decreased blood pressure are seen.

Precautions

It is contraindicated for patients with uveitis, neovascular glaucoma, peripheral retinal disease, retinal detachment, severe asthma, bronchial obstruction, etc.
