Function and Efficacy

This product belongs to the macrolide antibiotics, which is the dodecyl sulfate of erythromycin propionate. It has antibacterial activity against Staphylococcus, various groups of streptococci and Gram-positive bacilli. Neisseria, Haemophilus influenzae, Bordetella pertussis, etc. may also be sensitive to this product. This product also has antibacterial activity against various anaerobic bacteria except Bacteroides fragilis and Fusobacterium; it also has inhibitory effects on Legionella, Campylobacter fetus, some spirochetes, Mycoplasma pneumoniae, Rickettsia and Chlamydia. This product is an antibacterial agent, but it also has a bactericidal effect on some bacteria at high concentrations. This product can pass through the bacterial cell membrane and reversibly bind to the 50S subunit of the bacterial ribosome near the donor site ("P" site), blocking the transfer RNA (t-RNA) binding to the "P" site, and also blocking the displacement of the polypeptide chain from the acceptor site ("A" site) to the "P" site, thereby inhibiting bacterial protein synthesis. This product is only effective against bacteria that are actively dividing.

Indication

1. This product is used as an alternative to penicillin-allergic patients to treat the following infections: acute tonsillitis, acute pharyngitis, sinusitis caused by hemolytic streptococci, pneumococci, etc.; scarlet fever and cellulitis caused by hemolytic streptococci; diphtheria and diphtheria carriers; gas gangrene, anthrax, tetanus; actinomycosis; syphilis; listeriosis, etc. 2. Legionnaires' disease. 3. Mycoplasma pneumonia. 4. Chlamydia pneumoniae pneumonia. 5. Urinary and reproductive system infections caused by other chlamydia and mycoplasma. 6. Chlamydia trachomatis conjunctivitis. 7. Oral infections caused by anaerobic bacteria. 8. Campylobacter jejuni enteritis. 9. Whooping cough. 10. Preventive medication for recurrent rheumatic fever, infective endocarditis (after rheumatic heart disease, congenital heart disease, heart valve replacement surgery), oral and upper respiratory tract medical operations (alternative to penicillin).

Usage and Dosage

Oral administration: 0.75-2g per day for adults, divided into 3-4 times; 20-30mg/kg per day for children, divided into 3-4 times. For the treatment of Legionnaires' disease, 0.5-1.0g per time, 4 times a day for adults. When used as a preventive medication for recurrent rheumatic fever, 0.25g per time, 2 times a day. When used as a preventive medication for infective endocarditis, 1g is taken orally 1 hour before surgery, and 0.5g is taken 6 hours after surgery.

Adverse Reactions

1. Patients with hepatotoxic reactions after taking this product are more common than those taking other erythromycin preparations. After taking the drug for several days or 1 to 2 weeks, patients may experience fatigue, nausea, vomiting, abdominal pain, rash, fever, etc. Jaundice may sometimes occur, and liver function tests show cholestasis, which can often be recovered after stopping the drug. 2. Gastrointestinal reactions include diarrhea, nausea, vomiting, upper abdominal pain, mouth and tongue pain, and decreased appetite, and their incidence is related to the dose. 3. When used in large doses (≥4g/day), especially in patients with liver and kidney diseases or elderly patients, hearing loss may occur, which is mainly related to excessive blood drug concentration (12mg/L), and most of them can be recovered after stopping the drug. 4. Allergic reactions are manifested as drug fever, rash, eosinophilia, etc., with an incidence of about 0.5% to 1%. 5. Others: Occasionally there are arrhythmias and oral or vaginal candidal infections.

Precautions

It is contraindicated for patients who are allergic to erythromycin.

Special Population Medication

Precautions during pregnancy and lactation: This product can pass through the placenta and enter the fetal circulation. Pregnant women are more likely to experience hepatotoxic reactions after taking this product. Pregnant women should not use it. A considerable amount of erythromycin enters breast milk. Lactating women should stop breastfeeding when using this product.

Drug Interactions

1. This product can inhibit the metabolism of anti-epileptic drugs such as carbamazepine and valproic acid, resulting in increased blood concentrations of the latter and toxic reactions. This product can inhibit the metabolism of the latter and prolong its duration of action when used in combination with alfentanil. This product can increase cardiac toxicity when used in combination with antihistamines such as astemizole or terfenadine, and can increase the blood concentration of the latter and produce nephrotoxicity when used in combination with cyclosporine. 2. It has an antagonistic effect with chloramphenicol and lincosamides, and is not recommended for combined use. 3. This product is an antibacterial agent that can interfere with the bactericidal efficacy of penicillin. Therefore, when rapid bactericidal effects are required, such as the treatment of meningitis, the two should not be used in combination. 4. Patients who have been taking warfarin for a long time may experience a prolonged prothrombin time when using erythromycin, thereby increasing the risk of bleeding. Elderly patients should pay special attention. When the two must be used in combination, the dose of warfarin should be appropriately adjusted and the prothrombin time should be closely observed. 5. Except for dihydroxypropylphylline, the combination of this product with xanthines can reduce the hepatic clearance of aminophylline, leading to increased serum aminophylline concentrations and (or) increased toxic reactions. This phenomenon is more likely to occur after 6 days of combined use, and the reduction in aminophylline clearance is proportional to the serum peak value of this product. Therefore, the dose of xanthines should be adjusted during and after the combined treatment of the two. 6. Combination with other hepatotoxic drugs may enhance hepatotoxicity. 7. Large doses of this product combined with ototoxic drugs, especially in patients with renal impairment, may increase ototoxicity. 8. When used in combination with lovastatin, its metabolism can be inhibited and blood concentrations can be increased, which may cause rhabdomyolysis. When used in combination with midazolam or triazolam, the clearance of both can be reduced and their effects can be enhanced.
