Function and Efficacy

The antibacterial properties are similar to those of cefazolin, and it has good antibacterial effects on Staphylococcus (including enzyme-producing strains), Streptococcus pyogenes, Pneumococcus, Escherichia coli, Proteus mirabilis, Haemophilus influenzae, etc. This product is used orally, 0.25, 0.5 or 1g is taken on an empty stomach, and the peak blood concentration within 30 to 60 minutes is 7, 13 or 23mu;g/ml respectively. It is mainly distributed in the blood, internal organs, and skin tissues. The concentration in brain tissue is low. The half-life is 0.6 to 0.9 hours, and the drug is excreted in the urine in its original form. Taking 0.25g orally at a time, the peak urine concentration can reach 600mu;g/ml, and the half-life is slightly prolonged in patients with renal insufficiency. Cefaclor is a second-generation oral cephalosporin. Its antibacterial effect is stronger than that of cefadroxil. Its activity against group A hemolytic streptococci, viridans streptococci and Staphylococcus epidermidis is the same as that of cefadroxil. Its antibacterial activity against pneumococci and non-enzyme-producing Staphylococcus aureus is 2 to 4 times stronger than that of cefadroxil, and its activity against enzyme-producing Staphylococcus aureus is the same. Cefaclor is similar to cefadroxil in activity against Escherichia coli and Klebsiella pneumoniae; but its activity against Proteus mirabilis is significantly stronger than that of cefadroxil, with a MIC90 of 1.82 mg/L. The MIC of this product against Salmonella and Shigella is 0.25 to 1 mg/L, which is 8 times stronger than that of cefadroxil, and the MIC against gonococci is 0.016 to 1 mg/L. Many beta-lactamase-producing influenza bacilli are sensitive to this product; most common Proteus, Serratia, Acinetobacter and Pseudomonas aeruginosa are resistant to this product.

Indication

This product is clinically used for respiratory tract, urinary tract, skin, soft tissue infections, and otitis media caused by the above-mentioned sensitive bacteria. This product is currently the most widely used oral cephalosporin abroad. Nearly 2,000 cases of respiratory tract infections, urinary tract infections, and skin and soft tissue infections caused by sensitive bacteria have been treated with this product, with a clinical efficacy of 97% and a bacterial clearance rate of 90%. This product can be used for the above-mentioned infections caused by pneumococcus, group A hemolytic streptococci, staphylococci, influenza bacilli, Escherichia coli, some pneumonia bacilli, and Proteus mirabilis.

Usage and Dosage

Oral: The usual adult dose is 0.25g once, 3 times a day. The dose can be doubled for patients with severe infection, but the total daily amount should not exceed 4.0g. Or follow the doctor's advice.

Adverse Reactions

Can cause kidney damage. Long-term use can cause secondary infection. This product has few adverse reactions, with an incidence of about 3.4%. The most common are gastrointestinal reactions such as soft stools, diarrhea, stomach discomfort, loss of appetite, belching, etc., accounting for about 2.3%. Occasionally, allergic reactions such as itching and rash occur. About 0.3% of patients have elevated serum transaminase.

Precautions

Some patients may have cross-allergy to penicillin. Use with caution for those who are allergic to cephalosporins. Patients with mild renal insufficiency can be given the original dose. Patients with moderate and severe renal impairment can take 1/2 and 1/4 of the normal dose, respectively. Taking after meals reduces absorption and should be taken on an empty stomach. This product can pass through the placenta and should not be used by pregnant women.

Special Population Medication

Precautions during pregnancy and lactation: Not suitable for use by pregnant women.

Drug Interactions

The combined use of cefaclor and probenecid can reduce the renal excretion of the former and increase the blood concentration. When this product is used simultaneously with anticoagulants such as coumarin, patients may experience prolonged prothrombin time.
