Function and Efficacy

Appearance

This product is a colorless clear liquid.

Indication

1. Cardioversion of rapid paroxysmal supraventricular tachycardia. Vagus nerve inhibition (such as Valsalva method) should be the first choice before applying verapamil. 2. Temporary control of ventricular rate in atrial flutter or atrial fibrillation. Except when atrial flutter or atrial fibrillation is combined with atrioventricular bypass pathway (preexcitation syndrome and LGL syndrome).

Usage and Dosage

It must be injected slowly intravenously for at least 2 minutes under continuous ECG monitoring and blood pressure monitoring. This injection has no incompatibility with Ringer's solution, 5% glucose injection or sodium chloride injection. Because the optimal dosing interval for repeated intravenous administration cannot be determined, individualized treatment is required. The general starting dose is 5-10 mg (or 0.075-0.15 mg/Kg body weight), which is slowly injected intravenously for at least 2 minutes after dilution. If the initial reaction is not satisfactory, give another 5-10 mg or 0.15 mg/Kg body weight 15-30 minutes after the first dose. Intravenous drip administration, 5-10 mg per hour, added to sodium chloride injection or 5% glucose injection and dripped intravenously, the total amount per day should not exceed 50-100 mg.

Adverse Reactions

Take the recommended single dose and total daily dose as the starting dose and gradually adjust the dose upward. Serious adverse reactions are rare. Adverse reactions with an incidence of 1-10%: constipation (7.3%); dizziness, mild headache (3.5%); nausea (2.7%); hypotension (2.5%); headache (2.2%). Peripheral edema (2.1%); congestive heart failure (1.8%); sinus bradycardia, I, II or III degree atrioventricular block; rash (1.2%); fatigue; palpitations; increased transaminases, with or without increased alkaline phosphatase and bilirubin, which are sometimes transient and may disappear even with continued use of verapamil. Adverse reactions with an incidence of <1%: hypotension; tachycardia; flushing; galactorrhea; gingival hyperplasia; non-obstructive paralytic intestinal infarction

Precautions

1 Severe left ventricular dysfunction. 2 Hypotension (systolic blood pressure less than 90 mmHg) or cardiogenic shock. 3 Sick sinus syndrome (except patients with a pacemaker installed and functioning). 4 II or III degree atrioventricular block (except patients with a pacemaker installed and functioning). 5 Patients with atrial flutter or atrial fibrillation combined with atrioventricular bypass pathway. 6 Patients known to be allergic to verapamil hydrochloride.

Special Population Medication

Precautions for children: Non-controlled studies suggest that the effect of intravenous administration in neonates is similar to that in adults. However, a very small number of neonates and infants have serious and fatal hemodynamic side effects. Therefore, great caution must be exercised when administering medication to pediatric patients. The starting dose for children aged 0 to 1 years is 0.1 to 0.2 mg/kg body weight (usually 0.75 to 2 mg per dose), which is diluted and injected intravenously for at least 2 minutes under continuous ECG monitoring. If the initial response is not satisfactory, another 0.1 to 0.2 mg/kg body weight (usually 0.75 to 2 mg per dose) is given 30 minutes after the first dose under continuous ECG monitoring. 1 to 15 years old 0.1 to 0.3 mg/kg body weight (usually 2 to 5 mg per dose), with a total amount not exceeding 5 mg, which is injected intravenously for at least 2 minutes. If the initial response is not satisfactory, another 0.1 to 0.3 mg/kg body weight (usually 2 to 5 mg per dose) is given 30 minutes after the first dose. Precautions for pregnancy and lactation: Verapamil can pass through the placenta. It is only used for pregnant women who clearly need it and whose benefits outweigh the harm to the fetus. Verapamil can be secreted into breast milk, so breastfeeding should be discontinued during the use of verapamil. Precautions for the elderly: The initial dose for the elderly should be lower and should be administered slowly intravenously (at least 3 minutes).

Drug Interactions

1 Cytotoxic drugs such as cyclophosphamide, vincristine, procarbazine, prednisone, vinblastine, doxorubicin, and cisplatin reduce the absorption of verapamil. 2 Phenobarbital, hydantoin, vitamin D, sulfamethoxazole, and ramifluan reduce the plasma concentration of verapamil by increasing liver metabolism. 3 Cimetidine may increase the bioavailability of verapamil. 4 Verapamil inhibits the elimination of ethanol, leading to increased ethanol concentration in the blood, which may prolong the toxic effects of alcohol. 5 A few cases reported that the combined use of verapamil and aspirin prolonged bleeding time compared with aspirin alone. 6 Combined with beta-blockers
