Terlipressin for injection

Function and Efficacy

This drug is a precursor of lysine vasopressin. It is inactive by itself and acquires the physiological activity of vasopressin after being slowly converted into lysine vasopressin in the body. After injection into the blood, the glycyl group in its molecule is hydrolyzed by enzymes to produce a sustained low level of vasopressin. An appropriate amount of this drug can lower portal blood pressure, but it does not significantly change arterial blood pressure like vasopressin, nor does it increase the dissolution of fibrin. 2. Pharmacokinetics This drug takes effect 25-40 minutes after intravenous administration and lasts for 2-10 hours. The distribution half-life is 8-9 minutes and the distribution volume is 0.6-0.9L/kg. It is mainly metabolized in the liver and kidneys, and its metabolites are active lysine vasopressin. The elimination half-life is 51-66 minutes.

Indication

Suitable for bleeding esophageal varices.

Usage and Dosage

The conventional adult dose is 2 mg intravenously once, once every 4-6 hours, until the bleeding is controlled, for up to 24 hours. [Foreign Usage and Dosage Reference] The recommended dose for adult conventional intravenous injection is 2-4 mg, followed by a maintenance dose of 1 mg every 4-6 hours for 2-3 days. The maximum recommended daily dose is 0.12 mg/kg. Cardiovascular function and body fluid status should be closely monitored during medication.

Adverse Reactions

In some cases, abdominal colic, headache, short-term facial pallor, and increased arterial blood pressure may occur. [Foreign adverse reactions reference] 1. Cardiovascular system has been reported to cause orthostatic hypotension and increased arterial blood pressure in patients with Parkinson's disease. 2. Headache may occur in some cases of central nervous system. 3. Metabolic/endocrine system 1. Vasopressor effect without clinical significance may occur. 2. There are case reports that this drug can induce metabolic acidosis in the treatment of esophageal varices. 3. There are also reports that it can cause hypokalemia. 4. In some cases of digestive system, abdominal colic caused by increased intestinal motility and even fecal incontinence may occur. 5. Reproductive system significantly contracts the uterus, reducing uterine blood perfusion. 6. Respiratory system can cause bronchial contraction. 7. Musculoskeletal system has case reports of toxic rhabdomyolysis related to the use of this drug. 8. Skin Changes in skin blood flow lead to skin pallor within 5 minutes after intravenous injection, which is most severe at 30-45 minutes. This adverse reaction can last for 4 hours.
