Function and Efficacy
Theophylline can directly relax bronchial smooth muscle, especially in the bronchi that are in a state of contraction and spasm. It can also inhibit the release of histamine from mast cells and basophils, and has an anti-inflammatory effect. It can strengthen the contraction of the diaphragm and reduce fatigue, thereby improving the contraction of the diaphragm in patients with chronic obstructive pulmonary disease (COPD). Theophylline also enhances myocardial contraction and has a slight diuretic effect.

Appearance
This product is a capsule containing white spherical pellets.
Indication
⒈ Used to relieve or prevent chronic bronchial asthma or status asthmaticus in adults of all ages and children over 3 years old. Maintenance treatment after status asthmaticus can effectively prevent recurrence. Its efficacy is better than that of any single drug when used in combination with β2 receptor agonists or glucocorticoids, and its side effects are reduced and its tolerance is better. ⒉ It is also suitable for the symptoms of reversible bronchospasm associated with chronic bronchitis and emphysema. ⒊ It is a guarantee for the normal life of patients with chronic obstructive pulmonary disease (COPD) in winter, especially for asthma attacks at night.
Usage and Dosage
Swallow the whole capsule, or pour the small pills in half a spoon of warm water or liquid food and swallow. Adults take 0.2-0.3g once, once every 12 hours; children over 1 year old take 0.1-0.2g once (children aged 1-9 years old take 0.1g once; children aged 9-12 years old take 0.2g once; teenagers aged 12-16 years old take 0.2g once).
Adverse Reactions
Adverse reactions are related to the speed of theophylline clearance by individuals. Toxicity often occurs when the drug serum concentration is 15-20μg/ml. When theophylline blood concentration exceeds 20μg/ml in a few patients, headache, nausea, vomiting and insomnia are common, and indigestion, tremor and dizziness are less common. Most of them are mild to moderate, and severe ones are rare. When the blood concentration exceeds 40μg/ml, fever, dehydration, convulsions, etc. may occur, and in severe cases, even respiratory and cardiac arrest may occur.
Precautions
It is contraindicated for patients who cannot tolerate theophylline, patients with uncured latent epilepsy, and patients with acute myocardial infarction accompanied by hypotension.
Special Population Medication
Precautions for children: Children have a higher drug elimination rate, and a larger dose is required to achieve an effective blood drug concentration. The blood drug concentration should be monitored during medication. Precautions for pregnancy and lactation: Pregnant and lactating women should avoid using theophylline preparations. Precautions for the elderly: Elderly patients may have different elimination rates of theophylline, so the blood drug concentration needs to be tested and maintained at 7.5-12.5 mg/ml.
Drug Interactions
1. The combination of theophylline and ephedrine can increase the toxicity, and the combination with other sympathomimetic amine bronchodilators can also increase the toxicity. 2. The combination of theophylline with allopurinol (large dose), cimetidine, and oral contraceptives can increase the serum concentration of theophylline. 3. The combination of theophylline with rifampicin, phenobarbital, aminoglutethimide, pentobarbital and isoproterenol (intravenous injection) can reduce the serum concentration of theophylline; when theophylline is combined with phenytoin sodium, the blood concentrations of both are reduced. 4. For patients who need theophylline, it is best to avoid the use of non-selective beta-receptor blockers because their pharmacological effects antagonize each other. 5. When combined with clindamycin, lincomycin and certain macrolides (erythromycin, roxithromycin, clarithromycin) and quinolone antibacterial drugs (enoxacin, ciprofloxacin), the metabolism of this product in the liver can be reduced, the blood concentration can be increased, and even toxicity can occur. The dosage of this product should be adjusted before and after administration. 6 When used in combination with lithium salts, it can accelerate the excretion of lithium by the kidneys, which reduces the efficacy of the latter. 7 Smokers have enhanced liver metabolism of theophylline, so they need to increase the dosage.
