Recombinant human erythropoietin for injection (CHO cells)

Function and Efficacy

Erythropoietin is an active glycoprotein secreted by the kidneys. It acts on erythroid hematopoietic progenitor cells in the bone marrow and promotes their proliferation and differentiation. This product can induce a significant effect of stimulating colony formation through late-stage mother erythroid progenitor cells (CFUE). At high concentrations, this product can also stimulate early-stage mother erythroid progenitor cells (BFUE) to induce colony formation.

Indication

Anemia caused by renal insufficiency, including patients with chronic renal failure undergoing hemodialysis or non-dialysis treatment.

Usage and Dosage

This product should be used under the guidance of a physician. It can be injected subcutaneously or intravenously, 2 to 3 times a week. The dosage should be adjusted according to the patient's anemia degree, age and other related factors. The following is a recommended regimen for general treatment: Treatment period: The recommended initial dose is 100 to 150 U/kg body weight per week for hemodialysis patients and 75 to 100 IU/kg body weight per week for non-dialysis patients. If the increase in hematocrit is not ideal (less than 0.5 vol% per week), the dose can be increased by 15 to 30 IU/kg body weight after 4 weeks, but the maximum dose should not exceed 30 IU/kg body weight/week. The hematocrit should be increased to 30 to 33 vol%, but should not exceed 34 vol%. Maintenance period: If the hematocrit reaches 30 to 33 vol% or/and the hemoglobin reaches 100 to 110 g/L, the maintenance treatment phase will begin. It is recommended to adjust the dose to 2/3 of the treatment dose. Then check the hematocrit every 2 to 4 weeks to adjust the dose, avoid excessive red blood cell production, and maintain the hematocrit and hemoglobin at appropriate levels.

Adverse Reactions

1 General reaction: A few patients may experience headache, low fever, fatigue, etc. at the beginning of medication, and some patients may experience myalgia, joint pain, etc. Most adverse reactions can be improved after symptomatic treatment and will not affect continued medication. In very few cases, the above symptoms persist and medication should be discontinued. 2 Allergic reaction: A very small number of patients may experience allergic reactions such as rash or urticaria after medication, including anaphylactic shock. Therefore, when using this product for the first time or reusing this product, it is recommended to use a small amount first, and then inject the full amount after confirming that there is no abnormal reaction. If abnormalities are found, the drug should be discontinued immediately and properly handled. 3 Cardiovascular system: Increased blood pressure, worsening of existing hypertension, and headache, impaired consciousness, and spasm due to hypertensive encephalopathy may even cause cerebral hemorrhage. Therefore, during the treatment of erythropoietin injection, attention should be paid to and regular observation of blood pressure changes. If necessary, the dose should be reduced or discontinued, and the dose of antihypertensive drugs should be adjusted. 4 Blood system: With the increase of hematocrit, blood viscosity can increase significantly, so care should be taken to prevent thrombosis. 5 Liver: Occasionally, GOT and GPT may increase. 6 Gastrointestinal: Sometimes, nausea, vomiting, loss of appetite, diarrhea, etc. may occur.
