Carbenicillin Sodium for Injection
Function and Efficacy
This product is a broad-spectrum penicillin antibiotic that exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It has antibacterial effects on Enterobacteriaceae such as Escherichia coli, Proteus, Enterobacter, Citrobacter, Salmonella and Shigella, as well as other Gram-negative bacteria such as Pseudomonas aeruginosa, Haemophilus influenzae and Neisseria. It also has antibacterial activity against hemolytic streptococci, Streptococcus pneumoniae and Staphylococci that do not produce penicillinase. Many anaerobic bacteria such as Bacteroides fragilis and Clostridium are also sensitive to this product.

Appearance
This product is white crystalline powder.
Indication
It is mainly suitable for systemic Pseudomonas aeruginosa infections, such as sepsis, urinary tract infections, respiratory tract infections, abdominal and pelvic infections, as well as skin and soft tissue infections. It can also be used for systemic infections caused by other sensitive Enterobacteriaceae.
Usage and Dosage
This product can be used for intravenous drip and intravenous injection. For moderate infection, adults take 8g a day, divided into 2 to 3 intramuscular injections or intravenous injections, and children take 12.5 to 50mg/kg injections every 6 hours according to body weight; for severe infection, adults take 10 to 30g a day, divided into 2 to 4 intravenous drips or injections, and children take 100 to 300mg/Kg a day according to body weight, divided into 4 to 6 injections. For newborns weighing less than 2kg, the first dose is 100mg/Kg according to body weight, 75mg/kg every 12 hours in the first week after birth, intravenous drip; 100mg/kg from the second week after birth, once every 6 hours. For newborns weighing more than 2kg, 75mg/kg every 8 hours in the first week after birth, intravenous drip, and 75mg/kg every 6 hours thereafter. For patients with severe renal insufficiency, intravenous administration of 2g every 8 to 12 hours can maintain the blood drug concentration at the level of 100mg/L; if accompanied by liver damage, 2g a day is sufficient.
Adverse Reactions
1 Allergic reactions: Allergic reactions to penicillins are common, including various rashes such as urticaria, leukopenia, interstitial nephritis, asthma attacks, and serum sickness reactions (type III allergic reactions). In severe cases, anaphylactic shock may occasionally occur. Once anaphylactic shock occurs, it is necessary to rescue on the spot, keep the airway open, inhale oxygen, and use epinephrine, glucocorticoids and other treatment measures. 2 Gastrointestinal reactions: nausea, vomiting, hepatomegaly, etc., increased ALT, AST, and creatinine. 3 When this product is injected intravenously in large doses, nervous system reactions such as convulsions, hypernatremia and hypokalemia may occur. 4 This product is a weak acid, so acute metabolic acidosis may occur when the blood concentration is too high. This reaction is particularly common in patients with kidney disease and those who already have acidosis. 5 Others: Candida superinfection, bleeding, etc.
Precautions
It is contraindicated in patients with a history of allergy to penicillins or a positive penicillin skin test.
Drug Interactions
1 This product is contraindicated with succinic chloramphenicol, erythromycin ethylsuccinate, oxytetracycline hydrochloride, tetracycline hydrochloride, kanamycin, streptomycin, gentamicin, tobramycin, amphotericin B, vitamin B group, vitamin C, phenytoin sodium, sympathomimetic drugs, promethazine, etc. 2 This product has a synergistic antibacterial effect with aminoglycosides (amikacin, gentamicin or tobramycin) against Pseudomonas aeruginosa and some Enterobacteriaceae in vitro. 3 This product and aminoglycoside antibiotics drip in the same bottle can lead to a significant weakening of the antibacterial activity of both. 4 The combination of large doses of this product with anticoagulants such as heparin, thrombolytic drugs, salicylic acid preparations, sulfamethoxazole or platelet aggregation inhibitors can increase the risk of bleeding. 5 The combination with sulfonamides can increase the blood concentration of this product, so the dose of this product must be appropriately reduced.
