Xinfumectin Injection

Function and Efficacy

This product is a non-specific drug for the cell cycle and is most sensitive to the first half of the G1 phase, which is equivalent to the time of mRNA synthesis. Its mechanism of action is to bind to DNA, inhibit RNA polymerase using DNA as a template, and thus inhibit RNA synthesis. The binding method may be that its chromophore is embedded between the base pairs of DNA, while its peptide chain is located in the minor groove of the DNA double helix, hindering the RNA polymerase from moving along the DNA molecule. The inhibitory effect of this product on RNA synthesis is mainly the extension of the RNA chain rather than affecting its initiation. This product selectively binds to guanine in DNA and does not bind to RNA lacking guanine bases. This product cannot prevent DNA replication, because DNA polymerase can cause local denaturation of DNA (such as unwinding of the double helix) right in front of it, which can cause this product to dissolve faster.

Indication

Used for malignant hydatidiform mole, choriocarcinoma, and malignant lymphoma.

Usage and Dosage

Intravenous injection. The dosage depends on the patient's sensitivity to this product and the clinical use method. Systemic medication: The general dose is 300-400 μg per day (6-10 μg/kg per day), dissolved in 250-500 ml of 5% glucose solution, intravenous drip for 4-5 hours, 10-12 consecutive days as a course of treatment, and the total dose is generally about 3600-6000 μg. The second course of treatment is repeated about 2 weeks apart, and the daily dose can also be reduced as appropriate to extend the course of treatment. Combination medication: 1. Combined with vincristine and doxorubicin to treat Wilms tumor; 2. Combined with cyclophosphamide, vinblastine, bleomycin, and cisplatin to treat testicular tumors; 3. Combined with doxorubicin, cyclophosphamide, and vincristine to treat soft tissue sarcoma and Ewing sarcoma; It can also be used in combined chemotherapy regimens for the treatment of malignant lymphoma. 4. When used in combination with radiotherapy, it can increase the sensitivity of the tumor to radiotherapy, concentrate and retain in nucleated cells, and hinder radiorepair.

Adverse Reactions

1. Bone marrow suppression: White blood cells and platelets may decrease significantly, usually 7 to 14 days after medication. 2. Gastrointestinal reactions: Manifested as loss of appetite, nausea, vomiting, diarrhea, and a few oral ulcers. 3. Local reactions: Intravenous injection can cause phlebitis, and leakage outside the blood vessels can cause tissue necrosis. 4. Others: A few patients experience dizziness, fatigue, bone and joint pain, liver function damage, electrocardiogram changes, hair loss, etc.
