Function and Efficacy

It is an amide long-acting local anesthetic. Its anesthesia time is 2 to 3 times longer than that of lidocaine hydrochloride, and its diffusion is similar to that of lidocaine hydrochloride. It has little effect on circulation and respiration, is non-irritating to tissues, and does not produce methemoglobin. The commonly used dose has no effect on cardiovascular function. A large dose can cause a drop in blood pressure and a slow heart rate. It has a significant blocking effect on β-receptors. There is no obvious rapid tolerance. The maternal drug blood concentration is 4 times that of the fetus.

Indication

Used for local infiltration anesthesia, peripheral nerve block and intrathecal block.

Usage and Dosage

1 Brachial plexus block, 0.25% solution, 20-30ml or 0.375%, 20ml (50-75mg); 2 Sacral canal block, 0.25%, 15-30ml (37.5-75.0mg), or 0.5%, 15-20ml (75-100mg); 3 When blocking the epidural space, 0.25%-0.375% can relieve pain, and 0.5% can be used for general abdominal surgery, etc.; 4 Local infiltration, the total dosage is generally limited to 175-200mg (0.25%, 70-80ml), divided into doses within 24 hours, and the maximum daily dose is 400mg; 5 The total dosage of sympathetic ganglion block is 50-125mg (0.25%, 20-50ml); 6 Subarachnoid space block, the usual dosage is 5-15mg, and 10% glucose is added to form a high-density solution or diluted with cerebrospinal fluid to a nearly equal-density solution.

Adverse Reactions

1. A small number of patients may experience headache, nausea, vomiting, urinary retention and slow heart rate. If serious side effects occur, ephedrine or atropine can be injected intravenously. 2. Overdose or accidental entry into blood vessels can produce serious toxic reactions. Once myocardial toxicity occurs, there is little hope of recovery.

Precautions

This product is contraindicated for those who are allergic to it.

Drug Interactions

When used in combination with alkaline drugs, precipitation will occur and the drug will lose its effect.
