Triamterene Hydrochlorothiazide Tablets
Function and Efficacy
1. Effects on water and electrolyte excretion. (1) Diuretic effect. Urinary excretion of sodium, potassium, chloride, phosphorus and magnesium ions increases, while urinary excretion of calcium decreases. The mechanism of action of this type of drug is mainly to inhibit the reabsorption of sodium chloride by the distal tubule anterior segment and the proximal tubule (less severe), thereby increasing the Na[sup][/sup]-K[sup][/sup] exchange in the distal tubule and collecting duct, and increasing K[sup][/sup] secretion. Its mechanism of action is not yet fully understood. This type of drug can inhibit carbonic anhydrase activity to varying degrees, which can explain its effect on the proximal tubule. This type of drug can also inhibit phosphodiesterase activity, reduce the tubular uptake of fatty acids and mitochondrial oxygen consumption, thereby inhibiting the active reabsorption of Na[sup][/sup] and Cl[sup]-[/sup] by the tubule. (2) Antihypertensive effect. In addition to the diuretic and natriuretic effects, there may be extrarenal mechanisms involved in lowering blood pressure, which may be to increase the excretion of Na[sup][/sup] by the gastrointestinal tract. 2. Effects on renal hemodynamics and glomerular filtration function. Due to the reduced reabsorption of water and Na[sup][/sup] by the renal tubules, the increased pressure in the renal tubules, and the increased water and Na[sup][/sup] flowing through the distal convoluted tubules, the macula densa is stimulated through the tubule-glomerular reflex, which increases the secretion of renin and angiotensin in the kidney, causing renal vasoconstriction, decreased renal blood flow, contraction of the glomerular afferent and efferent arterioles, and decreased glomerular filtration rate. Triamterene: It can directly inhibit the Na[sup][/sup]-K[sup][/sup] exchange in the distal tubules and collecting ducts of the kidneys, thereby increasing the excretion of Na[sup][/sup], Cl[sup]-[/sup], and water, and reducing the excretion of K. When the two are used together, the diuretic effect is enhanced and the adverse reactions (hydrochlorothiazide causes hypokalemia while triamterene can retain potassium) are reduced.

This product is light yellow tablet.
Indication
Applicable to: 1. Edema diseases: excrete excess sodium and water from the body, reduce extracellular fluid volume, and eliminate edema. Common diseases include congestive heart failure, ascites due to liver cirrhosis, nephrotic syndrome, acute and chronic nephritis edema, early chronic renal failure, sodium and water retention caused by adrenocortical hormone and estrogen treatment; 2. Hypertension: It can be used alone or in combination with other antihypertensive drugs, mainly for the treatment of primary hypertension; 3. Central or nephrogenic diabetes insipidus; 4. Nephrolithiasis: It is mainly used to prevent stones formed by calcium salt components.
Usage and Dosage
Oral administration. 1 tablet at a time, 2 times a day. When used for hypertension, 1 tablet should be given once a day as a maintenance dose after blood pressure returns to normal.
Adverse Reactions
Most of the adverse reactions of hydrochlorothiazide are related to the dosage and course of treatment. 1. Side effects caused by water and electrolyte disorders are relatively common. Hypokalemia is more likely to occur, which is related to the potassium excretion effect of thiazide diuretics. Long-term potassium deficiency can damage the renal tubules. Severe potassium loss can cause vacuolar changes in the renal tubular epithelium and cause ectopic heart rates such as severe tachyarrhythmias. In hypochloremic alkalosis or hypochloremic and hypokalemic alkalosis, thiazides, especially hydrochlorothiazide, often significantly increase the excretion of chloride. In addition, hyponatremia is not uncommon, leading to central nervous system symptoms and aggravating renal damage. Dehydration causes a decrease in blood volume and renal blood flow, which can also cause a decrease in glomerular filtration rate. 1. Use with caution in patients with anuria or severe renal impairment; 2. Use with caution in patients with diabetes; 3. Use with caution in patients with hyperuricemia or a history of gout; 4. Use with caution in patients with severe liver damage; 5. Use with caution in patients with hypercalcemia; 6. Use with caution in patients with hyponatremia; 7. Use with caution in patients with lupus erythematosus; 8. Use with caution in patients with pancreatitis; 9. Use with caution in patients with sympathectomy; 10. Use with caution in infants with jaundice; 11. Use with caution in pregnant, postpartum and lactating women.
Special Population Medication
Precautions for children: Not clear. Precautions for pregnancy and lactation: Use with caution in pregnant women, parturients and lactating women. Precautions for the elderly: Elderly patients are more likely to develop electrolyte disorders and renal impairment when using this product, so it should be used with caution.
Drug Interactions
1. Adrenocortical hormone, adrenocorticotropic hormone, estrogen, amphotericin B (intravenous), sodium carbenoxolone, and licorice preparations can reduce the diuretic effect of this product. 2. Nonsteroidal anti-inflammatory analgesics, especially indomethacin, can reduce the diuretic effect of this product, and the nephrotoxicity increases when used together. 3. The diuretic and hypotensive effects of this product can be reduced when used in combination with sympathomimetic drugs. 4. The diuretic effect is enhanced when used in combination with dopamine. 5. When used in combination with antihypertensive drugs, the diuretic and hypotensive effects are enhanced. 6. Because the combination of hydrochlorothiazide and triamterene will further increase blood uric acid, the dose of the latter should be adjusted when used in combination with anti-gout drugs. 7. When used in combination with hypoglycemic drugs, the dose of the latter should be appropriately increased. 8. Cholestyramine can reduce the gastrointestinal absorption of hydrochlorothiazide, so it should be taken 1 hour after oral administration of cholestyramine.
