Heparin Calcium Injection

Function and Efficacy

This product is an anticoagulant that can affect many aspects of the coagulation process. This product accelerates the inactivation of AT-Ⅲ on coagulation factors by combining with antithrombin III (AT-Ⅲ) to form a complex, thereby inhibiting the formation of thrombin prokinase and counteracting the effect of the thrombin prokinase that has been formed. This product can inhibit the adhesion and aggregation of platelets, prevent platelet disintegration and release platelet factor 3 and 5-hydroxytryptamine. The anticoagulant effect of heparin calcium is related to the strong negatively charged sulfate group in its molecule. If the sulfate group is hydrolyzed or neutralized by protamine with a strong positive charge, it will quickly lose its anticoagulant activity. Recent studies have found that heparin calcium also has a variety of non-anticoagulant pharmacological effects such as regulating blood lipids, anti-inflammation, anti-complement, anti-allergy, and immune regulation.

Indication

Anticoagulants can inhibit the blood clotting process. They are used to prevent the formation of blood clots.

Usage and Dosage

Subcutaneous injection: 1 Adult dose: ① Deep subcutaneous injection, 5000-10000 units for the first time, 5000-10000 units every 8 hours or 10000-20000 units every 12 hours, or adjusted according to the results of coagulation test monitoring; ② Intravenous injection, 5000-10000 units for the first time, 50-100 units/kg every 4 hours according to body weight, or determined according to the results of coagulation test monitoring. Dilute with 50-100ml of sodium chloride injection before use; ③ Intravenous drip, 20000-40000 units per day, added to 1000ml of sodium chloride injection for 24 hours of continuous drip, usually 5000 units intravenous injection as the initial dose; ④ Prophylactic use, deep subcutaneous injection of 5000 units 2 hours before surgery, and then repeat the above dose every 8-12 hours for 7 days. 2. Pediatric dosage: ① Intravenous injection, the first dose is 50 units/kg based on body weight, and then 50-100 units/kg every 4 hours, or adjusted according to the results of coagulation test monitoring; ② Intravenous drip, the first dose is 50 units/kg, and then 50-100 units/kg, once every 4 hours, or 10,000-20,000 units/m2 based on body surface area, continuous drip for 24 hours, and can also be determined according to the results of the partial thromboplastin time (APTT or KPTT) test. For cardiovascular surgery, the first dose and the dosage for surgery lasting less than 60 minutes are the same as the usual dosage for adults. For disseminated intravascular coagulation, 25-50 units/kg are continuously intravenously dripped every 4 hours. If the condition does not improve after 4-8 hours, it should be discontinued.

Adverse Reactions

1. Local irritation: small nodules and hematomas may be seen at the injection site, which will disappear on their own after a few days. 2. Long-term use of the drug may cause bleeding, thrombocytopenia, osteoporosis, etc. 3. Allergic reactions are rare.
