Indication

It is a second-line treatment drug for pulmonary tuberculosis. It can be used as one of the combination drugs for patients who have failed to respond to first-line anti-tuberculosis drugs (such as streptomycin, isoniazid, rifampicin and ethambutol), or when the bacteria are toxic to one or more of the above drugs or are resistant to the drugs.

Usage and Dosage

Before use, add sodium chloride injection to dissolve. Deep intramuscular injection, adults 1g a day, continue for 60 to 120 days, and then 2 to 3 times a week, 1g each time; now it is recommended to take 0.75g each time, once a day.

Adverse Reactions

1. This product has significant nephrotoxicity, manifested as increased creatinine and urea nitrogen, decreased creatinine clearance, proteinuria, tubular urine, etc. Renal function and urine routine should be monitored during medication. 2. It has damage to the 8th cranial nerve. Generally, vestibular function damage may occur after 2 to 4 months of medication, while hearing damage is less common. 3. It has a certain neuromuscular blocking effect. 4. Allergic reactions such as rash, itching, and skin redness and swelling.

Precautions

It is contraindicated for those who are allergic to this product and pregnant women.

Drug Interactions

1. When used in combination with aminoglycosides, it may increase the possibility of ototoxicity, nephrotoxicity and neuromuscular blockade. Hearing loss may occur, which may be temporary and may continue to progress to deafness after drug withdrawal, often permanently. Neuromuscular blockade can lead to skeletal muscle weakness and respiratory depression or respiratory muscle paralysis, which can be helped by anticholinesterase drugs or calcium salts. 2. When used simultaneously or successively with amphotericin B, vancomycin, bacitracin, paromomycin, cyclosporine, carmustine, cisplatin, bumetanide, ethacrynic acid and furosemide, it may increase the possibility of ototoxicity and nephrotoxicity. Hearing and renal function tests should be performed regularly when used together. 3. Antihistamines, buclizine, cyclisone, meclizine, phenothiazines, thioxanthenes and trimethoprim-butyl amine may mask ototoxic symptoms such as tinnitus, dizziness or vertigo when used in combination with this product. 4 When used in combination with anti-neuromuscular blocking drugs, it can antagonize the latter's effects on skeletal muscles. Therefore, the dose of anti-myasthenic drugs needs to be adjusted at the time of or after combined use. 5 When methoxyflurane or polymyxin injections are used simultaneously or successively with this product, renal toxicity or neuromuscular blocking effects may increase, so combined use should be avoided; neuromuscular blocking effects can cause skeletal muscle weakness and respiratory depression or respiratory muscle paralysis; during or after surgery, this product should also be used with caution when used in combination with the two, and anticholinesterase drugs or calcium salts can help blockade recovery. 6 When this product is used in combination with opioid analgesics, the central respiratory depressant effects of the two may be additive, resulting in aggravated respiratory depression or prolonged depression or respiratory paralysis, and close observation and follow-up are required.
