Function and Efficacy

This product has high antibacterial activity against common aerobic Gram-positive bacteria, such as Staphylococcus aureus (including those resistant to penicillin G), Staphylococcus epidermidis, β-hemolytic Streptococcus, viridans Streptococcus and Streptococcus pneumoniae. It has good antibacterial effect on anaerobic bacteria, including Clostridium tetani, Corynebacterium diphtheriae and Clostridium perfringens. It is inactive against Gram-negative bacteria such as Enterococcus, meningococci, Neisseria gonorrhoeae and Haemophilus influenzae, as well as fungi. There is no cross-resistance between this product and penicillin, chloramphenicol, cephalosporins and tetracyclines, and there is partial cross-resistance with macrolides. Its mechanism of action is that the 50S subunit of the ribosome of sensitive bacteria prevents the extension of the peptide chain, thereby inhibiting the protein synthesis of bacterial cells. It is generally bacteriostatic, but at high concentrations, it also has a bactericidal effect on certain bacteria.

Appearance

This product is white crystalline powder; has a slight or special odor; tastes bitter.

Indication

This product is suitable for the treatment of severe infectious diseases caused by sensitive bacteria such as streptococci, pneumococci and Staphylococcus aureus. It can also be used to treat penicillin allergy or infectious diseases that clinical physicians believe are not suitable for penicillin. This product may cause the risk of pseudomembranous colitis caused by antibiotics. When choosing this drug, physicians should consider the infection and whether there is a possibility of using less toxic drugs (such as erythromycin). Lincomycin should not be used for mild bacterial infections or viral infections.

Usage and Dosage

Intramuscular injection Adults: 0.6g (1 vial) per day, twice a day. Children: (over one month old) 10-20 mg/kg (10,000-20,000 units/kg) per day according to body weight, divided into 2-3 injections. Intravenous drip Adults: 0.6g (1 vial) per day, dissolved in 100-200 ml (not less than 100 ml) infusion, intravenous drip for 1-2 hours (not less than 1 hour), once every 8-12 hours. Children: (over one month old) 10-20 mg/kg (10,000-20,000 units/kg) per day according to body weight, divided into 2-3 injections.

Adverse Reactions

1. Gastrointestinal reactions: symptoms such as nausea, vomiting, abdominal pain, diarrhea, etc.; severe cases may include abdominal cramps, abdominal tenderness, severe diarrhea (watery or purulent), accompanied by fever, abnormal thirst and fatigue (pseudomembranous colitis); diarrhea, enteritis and pseudomembranous colitis may occur at the beginning of medication or several weeks after discontinuation of medication. 2. Blood system: leukopenia, neutropenia, neutropenia and thrombocytopenia may occur occasionally; aplastic anemia is rare. 3. Allergic reactions: rash, itching, etc. may be seen, and urticaria, angioedema and serum sickness reactions may occasionally be seen; epidermal excoriation, bullous dermatitis, erythema multiforme and S-J syndrome have been reported rarely. 4. Occasionally, there have been reports of jaundice caused by the use of this product. 5. Hypotension, electrocardiogram changes and even cardiac and respiratory arrest may occur when this product is rapidly infused. 6. Intravenous administration may cause thrombophlebitis.

Precautions

1. Patients with a history of allergy to lincomycin and clindamycin are contraindicated; 2. This product can be concentrated in the fetal liver after passing through the placenta. Although there are no reports of problems in human use, the use of this product in pregnant women requires a full balance of pros and cons. This product can be secreted into breast milk, and lactating women should also use it with caution. If it must be used, breastfeeding should be suspended; 3. Infants under 1 month old should not use this product; 4. Elderly people with serious underlying diseases are prone to adverse reactions such as diarrhea or pseudomembranous colitis, and need to be closely observed during medication.

Special Population Medication

Precautions for children: Infants under 1 month old should not use this medicine. Precautions for pregnancy and lactation: Lactating women should also use this medicine with caution. If it must be used, breastfeeding should be suspended. Precautions for the elderly: Elderly people with serious underlying diseases are prone to adverse reactions such as diarrhea or pseudomembranous colitis, and need to be closely observed during medication.

Drug Interactions

1. It can enhance the neuromuscular blockade of inhaled anesthetics, leading to skeletal muscle weakness and respiratory depression or paralysis (apnea). Caution should be taken when used in combination during or after surgery. Treatment with anticholinesterase drugs or calcium salts is expected to be effective. 2. When used with antiperistaltic antidiarrheal drugs and antidiarrheal drugs containing white clay, this product may cause pseudomembranous colitis with severe watery diarrhea during the course of treatment or even a few weeks after the course of treatment. Because it can delay the excretion of colonic endotoxins, thereby causing prolonged and aggravated diarrhea, antiperistaltic antidiarrheal drugs should not be used in combination. When this product is used in combination with antidiarrheal drugs containing white clay, the absorption of the former will be significantly reduced, so the two should not be taken at the same time, and a certain time interval (at least 2 hours) is required. 3. This product has a neuromuscular blocking effect. When used in combination with anti-myasthenic drugs, the latter will weaken the effect on skeletal muscles. In order to control the symptoms of myasthenia gravis, the dose of anti-myasthenic drugs should be adjusted when used in combination. 4. Chloramphenicol or erythromycin can replace this product at the target site, or inhibit the latter from binding to the bacterial ribosome 50S subunit. In vitro tests show that lincomycin and erythromycin have antagonistic effects, so lincomycin should not be used in combination with chloramphenicol or erythromycin. 5. When used in combination with opioid analgesics, the respiratory depression of this product and the central respiratory depression of opioids may lead to prolonged respiratory depression or respiratory paralysis (apnea) due to cumulative phenomena, so patients must be closely observed or monitored. 6. This product can enhance the effects of neuromuscular blocking drugs, and the two should be avoided in combination. 7. It is contraindicated to use with neomycin and kanamycin when intravenously dripped in the same bottle. Because there may be significant antagonism in clinical application, these two drugs should not be used at the same time.
