Function and Efficacy
Netilmicin is a semi-synthetic aminoglycoside broad-spectrum antibiotic. Its mechanism of action is to inhibit the normal protein synthesis of sensitive microorganisms. This product has a wide range of antimicrobial effects, mainly targeting Gram-negative rods and a few Gram-positive bacteria, including Citrobacter, Enterobacter, Escherichia coli, Klebsiella, Morgan Proteus, Pseudomonas aeruginosa, Salmonella, Shigella and Staphylococcus (penicillin- and methicillin-resistant). This product is also active in vitro against some isolated Acinetobacter, Neisseria, indole-positive Proteus, Pseudomonas, and Serratia. Most streptococci and anaerobic microorganisms such as Clostridium are resistant to aminoglycosides.
Ingredients
The main ingredient of this product is netilmicin sulfate, whose chemical name is O-3-deoxy C-methyl-3-methylamino-beta;-L-arabinopyranosyl (1rarr;4)-O[2,6-diamino-2,3,4,6-tetradeoxy-alpha;-D-glyceryl-4-enehexopyranosyl-(1rarr;6)]-2-deoxy-N3-ethyl-L-streptamine sulfate.

Appearance
This product is white or light yellow powder or loose lumps.
Indication
It is suitable for the treatment of complicated urinary tract infections, sepsis, skin and soft tissue infections, abdominal infections and lower respiratory tract infections caused by sensitive bacteria.
Usage and Dosage
1. Patients with normal renal function: Adults use 3-4 mg/kg per day for complicated urinary tract infection, 4-6 mg/kg per day for moderate and severe infection, 6-7.5 mg/kg per day for severe systemic infection, and the dosage is given once every 8-12 hours. Pediatric dosage: The dosage for patients aged 6 weeks to 12 years is 4-6 mg/kg per day, and the dosage is given once every 8-12 hours; for patients under 6 weeks of age, the dosage is 4-5 mg/kg per day, and the dosage is given once every 12 hours. The dosage for premature infants and newborns should be appropriately reduced. Intravenous drip is appropriate for severe infection, newborns, and infants. The course of treatment varies depending on the disease and condition, and is generally 7-14 days. During the course of treatment with netilmicin, the patient's blood drug concentration should be monitored regularly to keep the peak blood drug concentration at 6-10 mg/L and the trough concentration at 0.5-2 mg/L. The blood drug concentration must be monitored when used in newborns. When intravenously dripping, each dose can be diluted with 50-200ml of sterile saline or glucose solution and then dripped for 1.5-2.0h. When used in children, the drug solution can be appropriately reduced and diluted according to the dose. 2. Patients with impaired renal function: When used, the dose must be adjusted according to the degree of renal function impairment. If conditions permit, blood drug concentration monitoring should be carried out, and an individualized dosing plan should be formulated based on the results to adjust the blood drug concentration to an appropriate range (see above). The dosage can also be reduced or the dosing interval can be extended based on the patient's endogenous creatinine clearance rate, or with reference to the blood creatinine value and blood urea nitrogen value.
Adverse Reactions
1. Nephrotoxicity: its toxicity is mild and rare. Nephrotoxicity often occurs in patients with existing renal impairment or those who are infected with doses exceeding the usual dose; 2. Nervous system toxicity: toxic reactions to the eighth cranial nerve may occur. Compared with other commonly used aminoglycoside antibiotics, its incidence is low and the degree is also mild. It is easy to occur in patients with existing renal impairment or infected patients with excessive treatment doses and long courses of treatment. It manifests as vestibular and hearing damage symptoms, such as dizziness and hearing abnormalities, but there are no reports of deafness; 3. Others: headache, general discomfort, visual impairment, palpitations, rash, fever, vomiting and diarrhea may occur occasionally; 4. Laboratory abnormalities may occur occasionally, such as increased blood sugar, blood alkaline phosphatase, serum transaminase, etc., and decreased white blood cells, platelets, etc. and increased eosinophils. The above reactions are transient; 5. Local reactions are generally rare, and there is occasional pain in the injection area.
Precautions
Patients who are allergic to this product or any aminoglycoside antibiotics or have severe toxic reactions are contraindicated.
Special Population Medication
Precautions for children: When children use this product, the pros and cons should be analyzed. This product is not recommended for newborns and premature infants. If it is necessary to use it, the dosage should be reduced or the administration time should be extended. Precautions for pregnancy and lactation: Aminoglycosides can enter the placenta and breast milk. Patients who use such drugs during pregnancy or lactation should be informed of the potential damage to the fetus or infant. For safety reasons, it is advisable to avoid using it. Precautions for the elderly: When using this product, reduce the dosage according to mild renal impairment.
Drug Interactions
Avoid co-administration with other nephrotoxic and neurotoxic drugs such as aminoglycoside antibiotics, vancomycin, polymyxin, strong diuretics, neuromuscular junction blockers, etc.
