Function and Efficacy

This product has an inhibitory effect on α-glycosidase, thereby inhibiting the hydrolysis of disaccharides and delaying the absorption of sugars. The concentration of 50% inhibition of the activity of α-glycosidase (maltase, sucrase) in the small intestine of pigs and rats is 10-8~10-9mol/L, which is 20~30 times that of the similar drug acarbose. This product competitively inhibits maltase, isomaltase and sucrase, and the inhibition constant is 1.5×10-9~7.0×10-9mol/L. When taken orally with various sugars, this product inhibits the increase in blood sugar after the intake of disaccharides (sucrose maltose) and starch, and has no effect on the increase in blood sugar after the intake of monosaccharides (glucose and fructose). When given to hyperglycemic rats at 1.78mg/(kg.d) for 4 weeks, blood sugar lowering effect appeared. At the same time, hypertriglyceridemia and hyperinsulinemia are alleviated. It can also inhibit the onset of hyperglycemia in rats fed with sucrose. Giving 9.53 mg/(kg.d) of this product to mice 4 weeks after birth inhibited the onset of hyperglycemia and hyperinsulinemia. Giving this product mixed with feed (at 0.67 mg/(kg·d)) to non-obese, non-insulin-dependent (NIDDM) diabetic rats can inhibit the increase of blood sugar after feeding. Giving the same rats mixed with feed at 3.58 mg/(kg·d) for 18 days can improve insulin secretion disorders and glucose tolerance. For NIDDM rats, the combination of insulin injection and oral administration of this product at 1.5 mg/(kg·d) can significantly reduce the urine sugar of rats that cannot be reduced by insulin alone at 4 U/(rat·day). Giving normal rats a single oral dose of this product and sucrose solution can inhibit blood sugar in a dose-dependent manner. The amount of sucrose in the digestive tract was not significantly different from that in the control group within 3 hours, but the amount and duration of sucrose remaining in the digestive tract in the 0.03 mg/kg group were prolonged, and about 1/3 of the sucrose dose in the 0.1 mg/kg group remained in the cecum and colon after 6 hours. The product was administered continuously for 28 days, and the disaccharide hydrolase in the small intestinal mucosa and the weight of the small intestinal mucosa increased, but these were reversible changes. The product was administered through the digestive tract and was hardly absorbed, and no other special effects were observed when administered intravenously.

Indication

Improve postprandial hyperglycemia in diabetes. Voglibose chewable tablets are suitable for patients who have not achieved significant results with diet therapy and exercise therapy, or who have not achieved significant results with oral hypoglycemic drugs or insulin preparations in addition to diet therapy and exercise therapy.

Usage and Dosage

Adults: 0.2 mg/time, 3 times a day, orally after the first bite of food. If the therapeutic effect is not obvious, the dosage can be increased to 0.3 mg each time after sufficient observation. There are reports of hypoglycemia when used in combination with insulin and sulfonylurea drugs. β-blockers, salicylic acid preparations, monoamine oxidase inhibitors, clofibrate hyperlipidemia treatment agents, and warfarin can enhance the hypoglycemic effect of diabetic drugs. Epinephrine, adrenocortical hormones, and thyroid hormones can reduce the hypoglycemic effect of diabetic drugs.

Adverse Reactions

Serious side effects: Hypoglycemia may occur when used in combination with other diabetes drugs. In addition, there are reports that hypoglycemia may occur occasionally without other diabetes drugs. This product can delay the digestion and absorption of disaccharides. When symptoms of hypoglycemia occur, sucrose should not be given but glucose should be given for treatment. Abdominal distension and increased flatulence may occur sometimes. Due to the increase in intestinal gas, intestinal obstruction-like symptoms may occasionally occur. Full observation should be made and appropriate treatment should be given if symptoms occur. Severe liver dysfunction such as jaundice, increased GOT and GPT may occur occasionally. In addition, there are reports that similar drugs (acarbose) have caused fulminant hepatitis, so full observation should be made and appropriate treatment should be given after symptoms occur. When given to severe cirrhosis cases, due to the worsening of hyperammonemia and impaired consciousness (frequency unknown) caused by constipation, etc., the bowel movement should be fully observed and abnormalities should be immediately treated appropriately.

Precautions

The following patients are prohibited from taking the drug: 1. Patients with severe ketosis, diabetic coma or pre-coma. [Since high blood sugar must be quickly adjusted with infusion and insulin, it is not suitable to take voglibose chewable tablets. 2. Patients with severe infection, patients before and after surgery, or patients with severe trauma. [Since it is necessary to adjust blood sugar by injection of insulin, it is not suitable to take voglibose chewable tablets. 3. Patients with a history of allergy to the ingredients of this product.

Special Population Medication

Precautions for children: The safety of the drug for children has not been established. Precautions for pregnancy and lactation: Pregnant women, parturients and lactating women should use the drug with caution, because the safety of the drug during pregnancy has not been established. Pregnant women or women who may become pregnant can only use the drug when it is determined that the benefits of treatment outweigh the risks. Although the use of the drug by lactating women should be avoided as much as possible, breastfeeding should be stopped when the drug has to be used. [Animal experiments (rats) have found that this product inhibits the increase in weight of newborn rats, presumably because it inhibits the absorption of sugar in the mother animal and inhibits milk secretion. Precautions for the elderly: The elderly usually have decreased physiological functions, so they should start with a small dose (for example, 0.1 mg once), and pay attention to the occurrence of blood sugar levels and digestive system symptoms, etc., and should use the drug with caution.

Drug Interactions
