Recombinant Streptokinase for Injection

The component of recombinant streptokinase for injection is recombinant streptokinase. Recombinant streptokinase combines with plasminogen in a 1:1 gram molecular ratio to form a complex, and then activates plasminogen into plasmin. Plasmin catalyzes the hydrolysis of fibrin, the main matrix of thrombus, thereby dissolving the thrombus and reopening the blood vessels. At the same time, the thrombolytic effect of recombinant streptokinase is enhanced by the presence of fibrin. Therefore, recombinant streptokinase can effectively and specifically dissolve thrombus or blood clots, and can treat diseases with thrombosis as the main pathological change.

Thrombotic diseases such as acute myocardial infarction.

Intravenous thrombolytic therapy for acute myocardial infarction: It is generally recommended that 1.5 million IU of this product be dissolved in 100 ml of 5% glucose and intravenously drip for 1 hour. Thrombolytic therapy for acute myocardial infarction should be started as early as possible, and treatment should be started within 12 hours of onset. For special patients (such as those who are too low in weight or significantly overweight), the doctor may increase or decrease the dose appropriately according to the specific situation (calculated at 20,000 IU/kg body weight).

1. Fever, chills, nausea and vomiting, shoulder and back pain, allergic rash; hypotension may occur during intravenous infusion of this product. If blood pressure drops, the infusion rate should be slowed down; anaphylactic shock is rare. Mild allergic reactions do not require interruption of treatment, and severe allergic reactions require immediate cessation of intravenous infusion. Allergic reactions can be treated with antihistamines or hormones; 2. Bleeding, bleeding at the puncture site, skin ecchymosis, gastrointestinal tract, urinary tract or respiratory tract bleeding; when recombinant streptokinase is used for thrombolytic therapy of acute myocardial infarction, the incidence of cerebral hemorrhage is 0.1-0.3%. In case of severe bleeding, 6-aminocaproic acid can be used, and fresh plasma or whole blood can be transfused; 3. Other reactions, when this product is used for thrombolytic therapy of acute myocardial infarction, reperfusion arrhythmia may occur, occasionally slow arrhythmia, accelerated ventricular spontaneous heart rate, ventricular premature beats or ventricular fibrillation, etc.; occasionally it may cause hemolytic anemia, jaundice and increased GPT; secondary embolism may occur after thrombolysis, such as pulmonary embolism, cerebral embolism or cholesterol embolism, etc.

1. Patients with a history of bleeding, surgery, trauma, cardiopulmonary resuscitation, or vascular puncture that cannot be performed with compression hemostasis within two weeks are contraindicated; 2. Patients with a history of ulcer bleeding, esophageal varices, ulcerative colitis, or hemorrhagic retinopathy within the past two weeks; 3. Uncontrolled hypertension, blood pressure above 180mmHg/110mmHg, or patients who cannot rule out aortic dissecting aneurysm; 4. Patients with coagulation disorders and hemorrhagic diseases; 5. Patients with severe liver and kidney dysfunction; 6. Patients with mitral stenosis combined with atrial fibrillation and left atrial thrombus (cerebral embolism may occur after thrombolysis), and infective endocarditis; 7. Pregnant women; 8. Patients allergic to streptokinase.

Precautions during pregnancy and lactation: Contraindicated for use by pregnant women.

It has a good effect when used together with aspirin to treat acute myocardial infarction. The use of anticoagulants or dextran beforehand may increase the risk of bleeding.
