Cefpodoxime Proxetil Capsules
Function and Efficacy
Cefpodoxime proxetil is an oral broad-spectrum third-generation cephalosporin. After entering the body, it is hydrolyzed into cefpodoxime by nonspecific esterase to exert antibacterial effects. It is effective against both Gram-positive and Gram-negative bacteria. The mechanism of action of this product is to achieve bactericidal effects by inhibiting the biosynthesis of microbial cell walls. This product is stable to beta-lactamase, so many beta-lactamase-producing microorganisms that are resistant to penicillin and cephalosporins are still sensitive to this product. This product is ineffective against some ultra-extended-spectrum beta-lactamases. In vitro and clinical infection studies have confirmed that this product is active against most of the following microorganisms: 1 Aerobic Gram-positive microorganisms: Staphylococcus aureus (including penicillinase-producing strains, but ineffective against methicillin-resistant Staphylococci), saprophytic Staphylococci, Streptococcus pneumoniae (except penicillin-resistant strains), and Streptococcus pyogenes. 2 Aerobic Gram-negative microorganisms: Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, Pseudomonas aeruginosa, Haemophilus influenzae (including beta-lactamase-producing strains), Moraxella catarrhalis, and Neisseria gonorrhoeae (including penicillinase-producing strains). This product is active against most of the following microorganisms in vitro, but its clinical significance is still unclear: 1 Aerobic Gram-positive microorganisms - Streptococcus agalactiae, Streptococcus (Groups C, F, and G). However, this product is ineffective against enterococci. 2 Aerobic Gram-negative microorganisms - Citrobacter heteromorphica, Klebsiella oxytoca, Proteus vulgaris, Providencia rettgeri, Haemophilus parainfluenzae. However, this product is inactive against most Pseudomonas and Enterobacter. 3 Anaerobic Gram-positive microorganisms - Peptostreptococcus spp.

Appearance
This medicine is a hard capsule containing white or off-white powder or granules.
Indication
This product is suitable for the following infections caused by sensitive bacteria: 1. Upper respiratory tract infections: for example: ear, nose and throat infections, including acute otitis media, sinusitis, tonsillitis and pharyngitis; 2. Lower respiratory tract infections: for example: community-acquired pneumonia, acute exacerbation of chronic bronchitis; 3. Simple urinary tract infections: for example: cystitis; 4. Simple skin and skin soft tissue infections: folliculitis (including purulent acne), furuncle, carbuncle, erysipelas, cellulitis, lymphangiitis (nodular) inflammation, suppurative paronychia, subcutaneous abscess, sweat glanditis, clustered acne, and combined infection of sebaceous cysts; 5. Acute simple gonococcal urethritis and cervicitis, and perianal inflammation caused by Neisseria gonorrhoeae.
Usage and Dosage
Oral, 2 tablets three times a day.
Adverse Reactions
Literature reports that the following adverse reactions may occur when using this product for treatment: 1. Gastrointestinal reactions: nausea, vomiting, diarrhea, soft stools, stomach pain, abdominal pain, loss of appetite or stomach discomfort, and occasionally constipation. 2. Allergies: If rash, urticaria, erythema, itching, fever, lymph node swelling or joint pain occurs, the drug should be discontinued and treated appropriately. 3. Blood: eosinophilia, thrombocytopenia, and occasionally granulocytopenia occur. 4. Liver: AST, ALT, ALF, LDH, etc. sometimes increase. 5. Kidney: BUN and Cr sometimes increase. 6. Bacteriological alternation: stomatitis and candidiasis are occasionally seen. 7. Vitamin deficiency: Vitamin K deficiency symptoms (hypoprothrombinemia, bleeding tendency, etc.) and vitamin B deficiency symptoms (glossitis, stomatitis, loss of appetite, neuritis, etc.) are occasionally seen. 8. Others: dizziness, headache, and edema are occasionally seen.
Precautions
1. Patients allergic to penicillin or β-lactam antibiotics are contraindicated. 2. Patients allergic to cefpodoxime are contraindicated.
Special Population Medication
Precautions for children: Safety and efficacy data for infants under 5 months old have not been established. Precautions for pregnancy and lactation: Although there is no experimental evidence that cefpodoxime proxetil affects embryo formation or teratogenesis, like all drugs, it should be used with caution in the first few months of pregnancy. Cefpodoxime proxetil can be secreted from human breast milk. To avoid adverse reactions in breastfeeding infants, breastfeeding women should stop breastfeeding or switch to other drugs. Precautions for the elderly: The pharmacokinetic parameters of cefpodoxime proxetil are not affected by age, but the distribution of patients with renal insufficiency is changed, the elimination half-life is extended by 1.5 to 2 hours, and the plasma clearance rate and renal clearance rate are reduced. Therefore, patients with moderate to severe renal insufficiency (creatinine clearance < 50ml/min) can reduce the dose by 50% or take the drug once a day.
Drug Interactions
Antacids or H2 receptor antagonists can reduce its absorption and lower its peak plasma concentration; probenecid can increase its plasma concentration level.
