Function and Efficacy
This product is a hypoglycemic drug. This product can reduce fasting and postprandial hyperglycemia in patients with type 2 diabetes, and HbAlc can be reduced by 1% to 2%. The mechanism of this product in lowering blood sugar may be: ① Increase the sensitivity of peripheral tissues to insulin and increase insulin-mediated glucose utilization; ② Increase the utilization of glucose by non-insulin-dependent tissues, such as the brain, blood cells, renal medulla, intestines, skin, etc.; ③ Inhibit hepatic gluconeogenesis and reduce hepatic glucose output; ④ Inhibit intestinal wall cells from taking up glucose; ⑤ Inhibit cholesterol biosynthesis and storage, and reduce blood triglyceride and total cholesterol levels. Unlike the effect of insulin, this product has no effect on promoting fat synthesis and no obvious hypoglycemic effect on normal people. It generally does not cause hypoglycemia when used alone for type 2 diabetes.

Appearance

Indication
It is used for patients with type 2 diabetes who are not satisfied with diet control alone, especially those who are obese and have hyperinsulinemia. This drug not only has the effect of lowering blood sugar, but also has the effect of reducing weight and hyperinsulinemia. It can be effective for patients with poor efficacy of certain sulfonylureas. For example, it is better to use it in combination with sulfonylurea hypoglycemic drugs, intestinal glycosidase inhibitors or thiazolidinedione hypoglycemic drugs than to use them alone.
Usage and Dosage
Oral administration: Adults start with 0.25g once, 2-3 times a day, and then adjust the dosage according to blood sugar and urine sugar levels, generally 1g-1.5g per day, and no more than 2g per day. Take half an hour before meals, enteric-coated tablets can reduce gastrointestinal reactions.
Adverse Reactions
1 Gastrointestinal reactions, manifested as loss of appetite, nausea, vomiting, diarrhea, stomach pain, metallic taste in the mouth. 2 Sometimes there is weakness, tiredness, weight loss, dizziness, and rash. 3 Although the incidence of lactic acidosis is very low, it should be paid attention to. Clinical manifestations include vomiting, abdominal pain, hyperventilation, mental disorders, and increased lactic acid concentration in the blood that cannot be explained by uremia, ketoacidosis, or salicylic acid poisoning. 4 It can reduce the intestinal absorption of vitamin B12, reduce hemoglobin, produce megaloblastic anemia, and can also cause malabsorption.
Precautions
The following conditions should not be used: 1. Type 2 diabetes accompanied by ketoacidosis, liver and kidney dysfunction, lung dysfunction, heart failure, respiratory failure, acute myocardial infarction, severe infection and trauma, major surgery, and clinical hypotension and hypoxia. 2. Alcoholics, excessive drinkers, dehydration, dysentery, malnutrition, and those who are allergic to this product and biguanides. 3. Diabetes combined with severe chronic complications. 4. Before intravenous pyelography or arterial angiography. 5. Patients with severe heart and lung diseases. 6. Patients with vitamin B12, folic acid or iron deficiency. 7. Patients with poor systemic condition.
Special Population Medication
Precautions for children: This experiment has not been conducted and there is no reliable reference. Precautions for pregnancy and lactation: It is forbidden. Precautions for the elderly: Elderly patients (65 years old) should use it with caution because of weakened renal function. The dosage should be reduced as appropriate. Lactic acidosis may occur, so it should be used with caution.
Drug Interactions
1. This product combined with insulin will enhance the hypoglycemic effect, so the insulin dose should be reduced. 2. It can enhance the anticoagulant effect of anticoagulants (such as warfarin), leading to bleeding tendency. 3. If this product is taken with alcoholic beverages, abdominal pain, acidosis and hypothermia may occur. 4. When this product is used in combination with sulfonylureas, it can cause hypoglycemia. 5. Cimetidine can increase the bioavailability of this product and reduce the renal clearance rate, so the dose of this product should be reduced.
