Function and Efficacy
Glucose is one of the main sources of heat for the human body. Each gram of glucose can produce 4 calories (16.7 kJ) of heat energy, so it is used to supplement heat. Treat hypoglycemia. When glucose and insulin are intravenously infused together, potassium ions are required for the synthesis of glycogen, so potassium ions enter the cells and the blood potassium concentration decreases, so it is used to treat hyperkalemia. Rapid intravenous push of hypertonic glucose injection has a tissue dehydrating effect and can be used as a tissue dehydrating agent. In addition, glucose is the main substance for maintaining and regulating the osmotic pressure of peritoneal dialysis fluid.

Indication
Replenish energy and body fluids, hypoglycemia, hyperkalemia, hypertonic solution used as tissue dehydrating agent, preparation of peritoneal dialysis solution, drug diluent, intravenous glucose tolerance test, for the preparation of GIK (polarizing solution) solution.
Usage and Dosage
1. When patients eat less or cannot eat for some reason, they can generally be given 25% glucose injection intravenously and replenish body fluids at the same time. The amount of glucose is calculated according to the required heat energy. 2. Glucose is the most important energy supply substance for total intravenous nutrition therapy. In non-protein heat energy, the ratio of glucose to fat heat supply is 2:1. The specific amount depends on the clinical heat needs. According to the needs of fluid replacement, glucose can be prepared into different concentrations of 25-50%, and insulin can be added when necessary. 1 unit of regular insulin is added for every 5-10g of glucose. Since the normal use of hypertonic glucose solution is more irritating to the vein and requires infusion of fat emulsion, large vein drip is generally used. 3. For severe hypoglycemia, 20-40ml of 50% glucose injection can be used for intravenous push. 4. For starvation ketosis, 5-25% glucose injection should be used for intravenous drip in severe cases. 100g of glucose per day can basically control the condition. 5. Dehydration isotonic dehydration is given by intravenous drip of 5% glucose injection. 6. Hyperkalemia can be treated with 10-25% injection, with 1 unit of regular insulin infused for every 2-4g of glucose, which can reduce serum potassium concentration. However, this therapy only allows extracellular potassium ions to enter cells, and the total potassium content in the body remains unchanged. If potassium excretion measures are not taken, hyperkalemia may recur. 7. Rapid intravenous injection of 20-50ml of tissue dehydration hypertonic solution (generally 50% glucose injection). However, the effect is short-lived. Clinically, attention should be paid to preventing hyperglycemia, and it is rarely used at present. When used to adjust the osmotic pressure of peritoneal dialysis fluid, 20ml of 50% glucose injection, i.e. 10g of glucose, can increase the osmotic pressure of 1L peritoneal dialysis fluid by 55mOsm/kgH2O.
Adverse Reactions
1. Phlebitis occurs when hypertonic glucose injection is dripped. If a large vein is used for dripping, the incidence of phlebitis decreases. 2. Extravasation of high-concentration glucose injection can cause local swelling and pain. 3. Reactive hypoglycemia: It is easy to occur when combined with excessive use of insulin, the original tendency to hypoglycemia, and the sudden cessation of total intravenous nutrition therapy. 4. Hyperglycemic non-ketotic coma: It is more common in patients with diabetes, stress, the use of large amounts of glucocorticoids, and uremia peritoneal dialysis patients who are given hypertonic glucose solutions and total nutrition therapy intraperitoneally. 5. Electrolyte imbalance: hypokalemia, hyponatremia, and hypophosphatemia are prone to occur when glucose is simply supplemented for a long time. 6. For those with existing heart failure, children and the elderly who are given too much and too fast fluid supplementation can cause palpitations, arrhythmias, and even acute left heart failure. 7. Hyperkalemia occasionally occurs in patients with type 1 diabetes when high-concentration glucose is used.
Precautions
It is contraindicated in patients with diabetes or severe heart failure complicated by edema.
