Description	Mirabegron (formerly YM-178, trade name Myrbetriq) is a drug for the treatment of overactive bladder. It was developed by Astellas Pharma and was approved in the United States in July 2012. Mirabegron activates the β3 adrenergic receptor in the detrusor muscle in the bladder, which leads to muscle relaxation and an increase in bladder capacity.

Mirabegron More Info

Mirabegron is a selective beta-3 adrenergic receptor agonist used to treat overactive bladder (OAB) with symptoms such as urinary urgency, urgency incontinence, and increased urinary frequency. It works by relaxing the detrusor smooth muscle of the bladder during the storage phase of the urinary bladder fill-void cycle, thereby increasing bladder capacity.

Mechanism of Action

Mirabegron activates the beta-3 adrenergic receptors in the bladder, which leads to relaxation of the detrusor muscle. This action helps to increase bladder capacity and reduce the frequency and urgency of urination.

Pharmacokinetics

Absorption: Mirabegron reaches maximum plasma concentrations (Cmax) approximately 3.5 hours after administration. Steady-state concentrations are achieved within seven days.

Distribution: It has a large volume of distribution (Vss 1670 L) and is bound to human plasma proteins, including albumin and alpha-1 acid glycoprotein.

Metabolism: Mirabegron is metabolized in the liver via multiple pathways, including dealkylation, oxidation, glucuronidation, and hydrolysis.

Excretion: The elimination half-life is approximately 50 hours in adults. It is primarily excreted through active tubular secretion and glomerular filtration.

Indications

Mirabegron is indicated for the treatment of OAB symptoms, including urgency, urgency incontinence, and increased urinary frequency. It is often used as an alternative to antimuscarinic medications, which can have broader side effects.

Dosage

Mirabegron is available in extended-release tablets, typically in doses of 25 mg or 50 mg. The usual starting dose is 25 mg once daily, which can be increased to 50 mg if needed.

Adverse Effects

Common side effects may include increased blood pressure, headache, and urinary tract infections. However, it generally has a better tolerance profile compared to antimuscarinic medications.

Contraindications
