Description	Product Overview Oxaliplatin is a third-generation platinum-containing antineoplastic agent, chemically described as [(1R,2R)-1,2-cyclohexanediamine-N,N ′ ][ethanedioato(2-)-O,O ′ ]platinum(II). It is supplied as an active pharmaceutical ingredient (API) for the manufacture of finished oncology dosage forms, and is also available as finished pharmaceutical preparations depending on the buyer's registration status. Since its introduction, oxaliplatin has become one of the most widely used cytotoxic agents worldwide and is included in standard treatment protocols and national essential medicine lists in many countries. This product is intended strictly for licensed pharmaceutical manufacturers, distributors, hospitals and research institutions. Mechanism of Action Oxaliplatin is a prodrug that requires intracellular activation. Through displacement and aquation reactions, the oxalate leaving group is removed to generate reactive diaminocyclohexane (DACH)-platinum species. These species bind preferentially to the N7 position of guanine and form intra-strand and, to a lesser extent, inter-strand DNA crosslinks, mainly guanine–guanine and adenine–guanine adducts. The resulting distortion of the DNA double helix blocks DNA replication and transcription, induces S-phase and G2/M cell-cycle arrest, and ultimately triggers apoptosis. A key feature that distinguishes oxaliplatin from cisplatin and carboplatin is its bulky, hydrophobic DACH carrier ligand. The DACH ring protrudes into the DNA major groove and is poorly recognised by the mismatch-repair (MMR) complex and by certain damage-recognition proteins. As a result, cytotoxic activity is largely independent of MMR status, replicative bypass is prevented, and there is a lack of complete cross-resistance with cisplatin and carboplatin. Oxaliplatin therefore retains activity in tumour models resistant to earlier platinum agents. Cellular uptake is additionally facilitated by organic cation transporters (OCT1/OCT2), which are highly expressed in colorectal cancer cells. Usage and Clinical Applications Oxaliplatin is a prescription cytotoxic agent used by qualified oncologists in the management of solid tumours. Its principal approved indications are: (1) adjuvant treatment of stage III colon cancer following complete resection of the primary tumour, and (2) treatment of advanced or metastatic colorectal cancer. It is also widely used in established protocols for gastric, oesophageal, pancreatic, biliary tract and hepatocellular malignancies, in both adjuvant and palliative settings, according to local labelling and clinical guidelines. Treatment decisions, patient selection, dose individualisation and monitoring are entirely the responsibility of the treating physician; the product must be prescribed, dispensed and used under professional supervision. Combination Therapy Oxaliplatin is rarely used as a single agent; its greatest value lies in combination regimens: Folinic acid/leucovorin + 5-fluorouracil + oxaliplatin), including FOLFOX4 and mFOLFOX6 variants — the reference adjuvant and first-line palliative backbone in colorectal cancer. Adverse Reactions and Safety Profile The most characteristic toxicity is peripheral sensory neuropathy, which presents in two forms. The acute form appears within hours to days of dosing as cold-triggered paraesthesia, dysaesthesia or numbness of the hands, feet and perioral region, sometimes with pharyngolaryngeal discomfort, jaw spasm or muscle cramps; it is transient and generally reversible. The chronic form is cumulative and dose-limiting, presenting as persistent distal sensory loss, tingling and functional impairment that may affect daily activities; it typically emerges after several treatment cycles and may improve slowly after discontinuation, though it can persist in some patients. Other commonly reported effects include gastrointestinal reactions (nausea, vomiting, diarrhoea, mucositis, constipation, anorexia), myelosuppression (neutropenia, thrombocytopenia, anaemia, usually mild to moderate), fatigue, fever, and hypersensitivity reactions ranging from rash and urticaria to bronchospasm and anaphylaxis. Hepatic effects such as transaminase elevation and sinusoidal vascular injury have been described. Compared with cisplatin, oxaliplatin causes markedly less nephrotoxicity and ototoxicity, although renal function should still be monitored. Rare events include immune-mediated haemolytic anaemia, thrombotic microangiopathy, interstitial lung disease and posterior reversible encephalopathy syndrome. Regular monitoring of full blood count, renal and hepatic function and neurological status is recommended throughout therapy. Oxaliplatin is contraindicated in patients with known hypersensitivity to platinum compounds and should not be used during pregnancy or breastfeeding; effective contraception is advised during treatment.
