Function and Efficacy

Cefoperazone is a third-generation cephalosporin. It has good antibacterial effects on Escherichia coli, Klebsiella, Proteus, Salmonella typhi, Shigella, Citrobacter and other Enterobacteriaceae and Pseudomonas aeruginosa, but has poor effects on Enterobacter aerogenes, Enterobacter cloacae, Salmonella typhimurium and Acinetobacter. Influenzae, Neisseria gonorrhoeae and Neisseria meningitidis are highly sensitive to this product. This product also has a good effect on all groups of streptococci and pneumococci, and has only a moderate effect on Staphylococcus aureus (methicillin-sensitive strains), and Enterococci are resistant. Cefoperazone has a good effect on most Gram-positive anaerobes and some Gram-negative anaerobes, and Bacteroides fragilis is resistant to this product. Cefoperazone has poor stability against most beta-lactamases. This product mainly inhibits the synthesis of bacterial cell walls.

Appearance

This product is white or off-white crystalline powder or freeze-dried block or powder; odorless; crystalline powder is hygroscopic, and freeze-dried products are easily hygroscopic.

Indication

It is suitable for various infections caused by sensitive bacteria such as pneumonia and other lower respiratory tract infections, urinary tract infections, biliary tract infections, skin and soft tissue infections, sepsis, peritonitis, pelvic infections, etc. The latter two should be used in combination with anti-anaerobic drugs.

Usage and Dosage

It can be used for intramuscular injection, intravenous injection or intravenous drip. Common dosage for adults: for general infection, 1-2g (2-4 sticks) once, once every 12 hours; for severe infection, 2-3g (4-6 sticks) once, once every 8 hours. For patients receiving hemodialysis, one dose should be replenished after dialysis. The daily dose for adults should not exceed 9g (18 sticks), but when immunodeficient patients have severe infections, the dose can be increased to 12g (24 sticks) per day. Common dosage for children: 50-200mg/kg per day, divided into 2-3 intravenous drips. To prepare intramuscular injection solution, add 2.8ml of sterile water for injection and 1ml of 2% lidocaine injection solution for every 1g of drug, and the concentration is 250mg/ml. For intravenous slow injection, add 40ml of glucose sodium chloride injection to dissolve every 1g of the drug; for intravenous drip, take 1-2g of cefoperazone and dissolve it in 100-200ml of glucose sodium chloride injection or other diluents, and the final drug concentration is 5-25mg/ml. The sodium content of each 1g of cefoperazone is 1.5mmol (34mg).

Adverse Reactions

1. Skin rash is more common, reaching 2.3% or more. 2. A small number of patients may also experience diarrhea, abdominal pain, eosinophilia, and mild neutropenia. 3. Temporary increase in serum aminotransferase, alkaline phosphatase, urea nitrogen or blood creatinine. 4. Thrombocytopenia, prolonged prothrombin time, etc. can be seen in individual cases. Occasional bleeding can be prevented or controlled with vitamin K. 5. Dysbacteriosis may occur in a small number of patients. 6. Those who drink alcohol or take alcoholic drugs or beverages during the use of this product may experience a disulfiram-like reaction.

Precautions

This product is contraindicated in patients who are allergic to cephalosporins or have a history of allergic shock and immediate reaction to penicillin.

Drug Interactions

1. This product has a synergistic effect on certain sensitive strains of Enterobacteriaceae and Pseudomonas aeruginosa when used in combination with aminoglycoside antibiotics (gentamicin and tobramycin). 2. This product can produce hypoprothrombinemia and thrombocytopenia. When used simultaneously with the following drugs, it may cause bleeding: anticoagulant heparin, coumarin or indandione derivatives, thrombolytic drugs, non-steroidal anti-inflammatory analgesics (especially aspirin, diflunisal or other salicylic acid preparations) and sulfinpyrazone, etc. 3. The chemical structure of this product contains a methylthiotetrazolyl side chain. Therefore, during the use of this product, drinking alcohol or intravenous injection of ethanol-containing drugs will inhibit the activity of acetaldehyde dehydrogenase, causing acetaldehyde accumulation in the blood, and causing disulfiram-like reactions such as drowsiness and hallucinations. Therefore, during the medication period and within 5 days after stopping the medication, patients cannot drink alcohol, take oral or intravenous injection of ethanol-containing drugs. 4. When this product is used in combination with aminoglycoside antibiotics, it cannot be dripped in the same bottle because it may affect the antibacterial activity of each other. 5. This product is contraindicated with the following drug injections: amikacin, gentamicin, kanamycin B, doxycycline, meclofenoxate, amarin (Bianmailing), diphenhydramine, potassium magnesium aspartate, hydroxyzine hydrochloride (Antalol), procainamide, aminophylline, prochlorperazine, cytochrome C, pentazocine (Zhentongxin), aprotinin, etc.
