Function and Efficacy
This product achieves bactericidal effect by inhibiting the synthesis of bacterial cell wall. In in vitro tests, this product has bactericidal effect on a wide range of common clinical bacteria and can resist hydrolysis by multiple beta-lactamases. Bacteria sensitive to this product include Gram-positive bacteria: Staphylococcus aureus (including strains that produce and do not produce penicillinase), Staphylococcus epidermidis, Streptococcus pneumoniae, Streptococcus pyogenes, and Streptococcus agalactiae. Gram-negative bacteria: Escherichia coli, Klebsiella, Enterobacter, Citrobacter, Haemophilus influenzae (including strains that produce and do not produce beta-lactamase), Proteus mirabilis, Proteus vulgaris, Morganella morganii, Providencia, Serratia, Salmonella, Shigella. Pseudomonas aeruginosa and some other Pseudomonas, some strains of calcoacetic Acinetobacter. Neisseria gonorrhoeae (including strains that produce and do not produce penicillinase), Neisseria meningitidis, Bordetella pertussis. Anaerobic bacteria: Peptococcus, Peptostreptococcus, Veillonella, Clostridium, Eubacterium, Lactobacillus, Fusobacterium, various strains of Bacillus fragilis, and other Bacteroides.

Indication
This product can be used to treat the following infections caused by sensitive bacteria: respiratory tract infections (upper and lower respiratory tract infections), urinary tract infections (upper and lower urinary tract infections), peritoneal infections (upper and lower urinary tract infections), peritonitis, cholecystitis, cholangitis and other intracavitary infections, sepsis, meningitis, skin and soft tissue infections, bone and joint infections. Pelvic inflammatory disease, endometritis, gonorrhea and other genital and tract infections. Prevention of infection: used to prevent infections caused by intra-abdominal, gynecological, cardiovascular and orthopedic surgeries.
Usage and Dosage
It can be used for intravenous and intramuscular injection (for the preparation method, please refer to the instructions for use). Common adult dosage: mild to moderate infection, 1-2g once, once every 12 hours; severe infection, 2-3g once, once every 8 hours. When receiving hemodialysis, a dose should be given after dialysis. The daily dose for adults generally does not exceed 9g, but when immunodeficient patients have severe infections, the dose can be increased to 12g per day. Common pediatric dosage: The dosage for infants has not yet been determined. Infants and children receive a daily dose of 25-200mg/kg according to body weight without adverse reactions, but the t1/2 time of newborns is prolonged, so the dose must be adjusted. When used to prevent surgical infection: 1 or 2g should be given intravenously 30-90 minutes before the start of surgery. This dose can be repeated every 12 hours. For most cases, the duration of preventive administration should not exceed 24 hours. For surgeries with a high possibility of infection (such as colorectal surgery) or surgeries with the possibility of severe infection (such as open heart surgery or repair arthroplasty), the prevention of this product can continue until 72 hours after the completion of the surgery. Intravenous injection: When intravenous infusion is used, first dissolve each gram of this product in 5ml sterile water for injection, and then add this solution to an appropriate injection diluent for intravenous infusion. When direct intravenous injection is used, the maximum dose of this product per injection is 2g for adults and 50mg/kg body weight for children. This drug should be diluted with an appropriate diluent to a final concentration of 100mg/ml, and the injection time should not be less than 3-5 minutes. Intramuscular injection: Sterile water for injection can be used to prepare intramuscular injection solution. When the injection solution concentration is above 250mg/ml, this solution should be prepared with 2% lidocaine hydrochloride injection. Deep intramuscular injection should be used for injection.
Adverse Reactions
Cefoperazone has low toxicity and is easily tolerated by patients. The incidence of side effects is about 4%, among which rash is the most common, reaching 2.3%. A small number of patients also have diarrhea, abdominal pain, eosinophilia, mild neutropenia, thrombocytopenia, thrombocytopenia, and dysbacteriosis. Those who drink alcohol or take alcohol-containing drugs during the use of this product may experience a disulfide-like reaction.
Precautions
This product is contraindicated in patients with known allergy to cephalosporin antibiotics.
Drug Interactions
1. This product has a synergistic effect on certain sensitive strains of Enterobacteriaceae and Pseudomonas aeruginosa when used in combination with aminoglycoside antibiotics (gentamicin and tobramycin). 2. This product can produce hypoprothrombinemia and thrombocytopenia. When used simultaneously with the following drugs, it may cause bleeding: anticoagulant heparin, coumarin or indandione derivatives, thrombolytic drugs, non-steroidal anti-inflammatory analgesics (especially aspirin, diflunisal or other salicylic acid preparations) and sulfinpyrazone, etc. 3. The chemical structure of this product contains a methylthiotetrazolyl side chain. Therefore, during the use of this product, drinking alcohol or intravenous injection of ethanol-containing drugs will inhibit the activity of acetaldehyde dehydrogenase, causing acetaldehyde accumulation in the blood, and causing disulfiram-like reactions such as drowsiness and hallucinations. Therefore, during the medication period and within 5 days after stopping the medication, patients cannot drink alcohol, take oral or intravenous injection of ethanol-containing drugs. 4. When this product is used in combination with aminoglycoside antibiotics, it cannot be dripped in the same bottle because it may affect the antibacterial activity of each other. 5. This product is contraindicated with the following drug injections: amikacin, gentamicin, kanamycin B, doxycycline, meclofenoxate, amarin (Bianmailing), diphenhydramine, potassium magnesium aspartate, hydroxyzine hydrochloride (Antalol), procainamide, aminophylline, prochlorperazine, cytochrome C, pentazocine (Zhentongxin), aprotinin, etc.
