Function and Efficacy

Aminoglycoside antibiotics. This product has good antibacterial effects on most Enterobacteriaceae such as Escherichia coli, Klebsiella, Enterobacter, Proteus, Shigella, Salmonella, Citrobacter, Providencia, Yersinia, etc.; Haemophilus influenzae, Brucella, Neisseria meningitidis, Neisseria gonorrhoeae, etc. are also mostly sensitive to this product. Kanamycin also has a certain effect on Staphylococcus (methicillin-sensitive strains) and Mycobacterium tuberculosis, but is ineffective against Pseudomonas aeruginosa. Its antibacterial mechanism is to bind to the 30S subunit of the bacterial ribosome and inhibit bacterial protein synthesis. Other Gram-positive bacteria such as hemolytic streptococci, Streptococcus pneumoniae, Enterococcus and anaerobic bacteria are mostly resistant to this product. In recent years, the number of resistant strains has increased significantly, because some bacteria produce aminoglycoside inactivating enzymes, which make them lose their antibacterial activity.

Usage and Dosage

For oral use in intestinal infections, 0.75-1.25 g (750,000-1.25 million units) is taken 3-4 times a day. For children, take 40 mg (40,000 units) per kg of body weight per day, divided into 4 doses.

Adverse Reactions

1. Gastrointestinal reactions such as nausea and diarrhea may occur. 2. There are reports of irreversible hearing loss and hearing impairment in children after use.

Precautions

Patients with a history of allergy to this product or other aminoglycosides are contraindicated.

Special Population Medication

Precautions for children: This product belongs to the aminoglycoside class of drugs, which has nephrotoxic and ototoxic effects and should be used with caution in pediatrics. It should not be used in premature infants and newborns because their kidney tissue is not fully developed, which prolongs the half-life of this type of drug. The drug may accumulate in the body and produce toxic reactions. Precautions for pregnancy and lactation: This product can cross the placenta and enter the fetal tissue, and may cause hearing damage to the fetus. The secretion amount of this product in breast milk is very low, but breastfeeding should usually be suspended during medication. Drug Interactions

1. When used in combination with other aminoglycosides or when used systemically one after the other, it may increase ototoxicity, nephrotoxicity and neuromuscular blocking effects. 2. When used in combination with neuromuscular blocking drugs, it may aggravate neuromuscular blocking effects, leading to muscle weakness, respiratory depression, etc. 3. When used in combination with capreomycin, cisplatin, ethacrynic acid, furosemide or vancomycin (or norvancomycin), or when used systemically one after the other, it may increase ototoxicity and nephrotoxicity. 4. When used topically or systemically with cephalothin or cefazolin, it may increase nephrotoxicity.
