Azithromycin Capsules

Function and Efficacy

Azithromycin inhibits bacterial protein synthesis by hindering the bacterial transpeptidation process. In vitro tests have shown that azithromycin is effective against a variety of common clinical pathogens, including Gram-positive aerobic bacteria: Staphylococcus aureus, Streptococcus pyogenes (Group A β-hemolytic streptococci), Pneumococcus, α-hemolytic streptococci (grass viridans streptococcus group) and other streptococci, diphtheria (rod-shaped) bacilli. Azithromycin shows cross-resistance to erythromycin-resistant Gram-positive bacteria, including Streptococcus faecalis (Enterococcus) and methicillin-resistant Staphylococcus aureus strains. Gram-negative aerobic bacteria: Haemophilus influenzae, Haemophilus parainfluenzae, Catarrhalis, Acinetobacter, Yersinia, Legionella pneumophila, Bordetella pertussis, Bordetella parapertussis, Shigella, Pasteurella, Vibrio cholerae, Parahaemolyticus, Shigella-like Pseudomonas. Anaerobic bacteria: Bacteroides fragilis, Bacteroides, Clostridium perfringens, Peptostreptococcus, Fusobacterium necroticum, Propionibacterium acnes. Sexually transmitted disease microorganisms: Chlamydia trachomatis, Treponema pallidum, Neisseria gonorrhoeae, Haemophilus dukeii. Other microorganisms: including Borrelia tenens (Lyme disease agent), Mycoplasma pneumoniae, Mycoplasma hominis, Ureaplasma, Pneumocystis carinii, Mycobacterium avium, Campylobacter, Listeria monocytogenes. The following Gram-negative bacteria are usually resistant: Proteus, Serratia, Morganella, Pseudomonas aeruginosa. Animal model studies have found that this product exists in phagocytes, and when phagocytes are activated, high concentrations of azithromycin are released, so this product can reach high concentrations at the site of infection. About 12% of the intravenous dose is excreted in the urine as the original drug within 3 days, and most of it is excreted within the first 24 hours. In addition, this product can be excreted through the bile duct in its original form (high concentrations of azithromycin can be seen in bile) and 10 metabolites. It has been confirmed that the metabolites have no antibacterial activity.

Simple genital infections caused by non-multi-drug resistant gonococci (co-infection with Treponema pallidum must be excluded).

Usage and Dosage

The course of treatment and use of azithromycin capsules for infectious diseases are as follows: Adults: For sexually transmitted diseases caused by Chlamydia trachomatis or sensitive gonococci, only 1.0g of this product is needed orally at a single time. For the treatment of other infections: the total dose is 1.5g, divided into three doses; take 0.5g of this product once a day. Or the total dose is the same, still 1.5g, take 0.5g on the first day, and then take 0.25g of this product orally once a day from the second to the fifth day.

Special Population Medication

Precautions for children: The efficacy and safety of the drug in treating otitis media, community-acquired pneumonia in children under 6 months old, and pharyngitis or tonsillitis in children under 2 years old have not been determined. Precautions for pregnancy and lactation: Animal experiments have shown that this product has no effect on the fetus, but there is still a lack of experience in its use in pregnant women, so the pros and cons must be fully weighed before using it in pregnant women. Precautions for the elderly: This experiment has not been conducted and there are no reliable references.

Drug Interactions

(1) It is not advisable to take it at the same time with antacids containing aluminum or magnesium, as the latter can reduce the peak blood concentration of this product; if it must be taken together, this product should be given 1 hour before or 2 hours after taking the above drugs. (2) When taken together with theophylline, the concentration of the latter in the plasma can be increased, and the plasma theophylline level should be carefully tested. (3) When taken together with warfarin, the prothrombin time should be carefully checked. (4) When taken together with the following drugs, it is recommended to closely observe the patient: digoxin - increases digoxin levels; ergotamine or dihydroergotamine - acute ergot toxicity, symptoms are severe peripheral vasospasm and dysesthesia (pain when touching objects); triazolam - by reducing the degradation of triazolam, the pharmacological effect of triazolam is enhanced; cytochrome P450 system metabolizers - increase the levels of carbamazepine, terfenadine, cyclosporine, cyclohexylbarbital, and phenytoin in the serum (5) When taken together with rifabutin, the latter's toxicity will be increased.
