Function and Efficacy
This product is a compound preparation of gliclazide and metformin hydrochloride, and the two drugs have complementary mechanisms of action. The effect of metformin hydrochloride is to increase the sensitivity of the liver and peripheral tissues to insulin, and to improve the uptake and utilization of glucose. It may also have the effect of improving blood lipid levels and fibrinolytic protein activity. Gliclazide can stimulate pancreatic β cells to secrete insulin. Its mechanism of action is to specifically bind to the sulfonylurea receptors on the β cell membrane, thereby closing the potassium ion channel and causing changes in membrane potential. As a result, the calcium ion channel opens, and the calcium ions in the cytosol increase, prompting the secretion of insulin. Indication
This product is used as an alternative treatment when patients with type 2 diabetes require combined treatment with gliclazide tablets 40 mg and metformin hydrochloride tablets 250 mg.
Usage and Dosage
Oral administration: Adults start with 0.25g once, 2-3 times a day, and then adjust the dosage according to blood sugar and urine sugar levels, generally 1g-1.5g per day, and no more than 2g per day. Take half an hour before meals, enteric-coated tablets can reduce gastrointestinal reactions.
Adverse Reactions
1. Gastrointestinal reactions, manifested as loss of appetite, nausea, vomiting, diarrhea, stomach pain, metallic taste in the mouth. 2. Sometimes there is weakness, tiredness, weight loss, dizziness, and rash. 3. Although the incidence of lactic acidosis is very low, it should be paid attention to. Clinical manifestations include vomiting, abdominal pain, hyperventilation, mental disorders, and increased lactic acid concentration in the blood that cannot be explained by uremia, ketoacidosis, or salicylic acid poisoning. 4. It can reduce the intestinal absorption of vitamin B12, reduce hemoglobin, produce megaloblastic anemia, and can also cause malabsorption.
Precautions
1. This product is contraindicated in patients with kidney disease or the following conditions: renal dysfunction caused by heart failure (shock), acute myocardial infarction and sepsis (serum creatinine level ≥1.5 mg/dL (male), ≥1.4 mg/dL (female) or abnormal creatinine clearance). 2. Congestive heart failure requiring drug treatment, and other serious heart and lung diseases. 3. Severe infection and trauma, major surgical operations, clinical hypotension and hypoxia, etc. 4. Known allergy to metformin hydrochloride. 5. Acute or chronic metabolic acidosis, including diabetic ketoacidosis with or without coma, and diabetic ketoacidosis requiring insulin treatment. 6. Special Population Medication
Precautions for children: Children are prohibited from taking this product. Precautions for pregnancy and lactation: Precautions for pregnant and lactating women. Precautions for the elderly: Renal function declines with age. Elderly patients should monitor their renal function at all times when using this product. Generally, elderly patients should not take the highest dose of this product.
Drug Interactions
Thiazides and other diuretics, corticosteroids, phenothiazines, thyroid preparations, sex hormones, oral contraceptives, phenytoin, niacin, sympathomimetics, calcium channel blockers, and isoniazid can cause difficult-to-control hyperglycemia. When patients take this product and the above drugs at the same time, they should pay attention to changes in the patient's blood sugar level. When stopping taking the above drugs, pay attention to whether hypoglycemia occurs. Some protein inhibitors (such as salicylates, sulfonamides, chloramphenicol, probenecid, etc.) have little effect on metformin. Sulfonylureas have a high binding rate to serum proteins and are more likely to interact with these drugs than metformin. Nonsteroidal anti-inflammatory drugs, other strong protein inhibitors, salicylates, and sulfonamides can enhance the blood sugar-lowering effect of sulfonylureas.
