Function and Efficacy

This product is an antifibrinolytic drug. Fibrinogen specifically binds to fibrin through the lysine binding site in its molecular structure, and then turns into plasmin under the action of activators. This enzyme can cleave the arginine and lysine peptide chains in fibrin to form fibrin degradation products, which dissolve blood clots. The chemical structure of this product is similar to lysine, which can qualitatively inhibit the binding of plasminogen to fibrin and prevent its activation, thereby inhibiting fibrinolysis. High concentrations (100 mg/L) directly inhibit the activity of plasmin to achieve a hemostatic effect.

Appearance

This product is a colorless or almost colorless clear liquid.

Indication

It is suitable for the prevention and treatment of various bleeding caused by hyperfibrinolysis. ⑴ Bleeding caused by trauma or surgery in organs rich in plasminogen activators such as the prostate, urethra, lungs, liver, pancreas, brain, uterus, adrenal glands, thyroid glands, and bleeding caused by excessive tissue plasminogen activator (t-PA), streptokinase or urokinase. ⑵ Late stage of disseminated intravascular coagulation (DIC) to prevent secondary hyperfibrinolysis. ⑶ It can be used as an auxiliary treatment for bleeding after tooth extraction or oral surgery or menorrhagia in hemophilia patients. ⑷ It can be used for symptomatic treatment of various bleeding such as upper gastrointestinal bleeding, hemoptysis, idiopathic thrombocytopenic purpura and leukemia. It has a significant effect on general chronic oozing; it has poor efficacy on bleeding caused by abnormal coagulation function; it is effective for severe bleeding, heavy bleeding from wounds and bleeding from cancer, etc.

Usage and Dosage

Because this product is excreted quickly, continuous administration is required to maintain effective concentration, so it is generally used for intravenous drip. The effective concentration of this product in vivo to inhibit fibrinolysis is at least 130μg/ml. For patients with surgical bleeding or massive internal bleeding, rapid hemostasis requires rapid achievement of the above blood concentration. The initial amount can be 4-6g (20% solution) dissolved in 100ml of saline or 5-10% glucose solution, and dripped in 15-30 minutes. The continuous dose is 1g per hour, which can be taken orally or injected. Maintain 12-24 hours or longer, depending on the condition.

Adverse Reactions

⑴ This drug has certain side effects. As the dosage increases, the number of adverse reactions increases, the symptoms worsen and the duration of efficacy is shorter. It is now gradually used less. ⑵ Common adverse reactions are nausea, vomiting and diarrhea, followed by dizziness, itching, tinnitus, tinnitus, general discomfort, nasal congestion, rash, erythema, and no ejaculation. This is especially likely to occur when the daily dose exceeds 16g. ⑶ Postural hypotension, conjunctival and nasal mucosal congestion, etc. may occur due to vasodilation. ⑷ This product is excreted quickly in the urine and has a high urine concentration. It can inhibit the fibrinolytic effect of urokinase, form blood clots, and block the urinary tract. Therefore, patients with hematuria after urology surgery should use it with caution. ⑸ Thrombosis and heart disease are prone to occur

Precautions

⑴; Use with caution in patients with bleeding after urethral surgery; ⑵; Avoid use in patients with thrombotic tendency or past vascular embolism; ⑶; Use with caution in patients with renal insufficiency.

Drug Interactions

⑴ This product has poor immediate hemostatic effect and should be used in combination with other hemostatic drugs for acute massive bleeding. ⑵ This product should not be mixed with hemostatic drugs for injection.
